More than 19 million Americans have diabetes—a condition that
can produce life-threatening complications. Of the two major
forms of diabetes—type 1 and type 2—type 2 comprises 90 to 95 percent
of all cases in the United States. An additional 13 million people
have the precursor to type 2 diabetes, called prediabetes. Worldwide,
type 2 diabetes affects more than 190 million people, and some experts
predict that if the current trends continue that figure could surge to
over 300 million by the year 2025....
This book is written with the intention of providing nurse practitioners working in
the field of acute medicine with an up-to-date, practical and comprehensive guide to
the management of acute medical patients.
It is hoped that it will serve as a text from which the busy, highly skilled nurse can
obtain information on the assessment, investigation, diagnosis and management of
acute medical conditions.
In my role as Consultant Nurse in Acute Medicine I appreciate the diversity this
speciality brings and the challenges faced by working at an advanced level in this
Most patients are anemic and thrombocytopenic at presentation. Replacement of the appropriate blood components, if necessary, should begin promptly. Because qualitative platelet dysfunction or the presence of an infection may increase the likelihood of bleeding, evidence of hemorrhage justifies the immediate use of platelet transfusion, even if the platelet count is only moderately decreased.
About 50% of patients have a mild to moderate elevation of serum uric acid at presentation.
Treatment of Promyelocytic Leukemia
Tretinoin is an oral drug that induces the differentiation of leukemic cells bearing the t(15;17). APL is responsive to cytarabine and daunorubicin, but about 10% of patients treated with these drugs die from DIC induced by the release of granule components by dying tumor cells. Tretinoin does not produce DIC but produces another complication called the retinoic acid syndrome. Occurring within the first 3 weeks of treatment, it is characterized by fever, dyspnea, chest pain, pulmonary infiltrates, pleural and pericardial effusions, and hypoxia.
Cute pelvic pain is a common presenting com-plaint in women. The diagnosis of pelvic pain in women can be challenging because many symp-toms and signs are insensitive and unspecific. Prompt diagnosis and effective management pre-vent complications and may help preserve fertility .
This book describes different perspectives of childhood acute lymphoblastic leukemia. The approach includes aspects of molecular epidemiology, particularly molecular features that influence the genesis and prognosis of the disease. Some aspects of the prognosis of lymphoblastic leukemias are very detailed, highlighting the use of molecular biology in the early identification of complications that may occur in diseased patients.
Diabetes mellitus and its complications are clinical conditions of growing
importance both from the clinical as well as epidemiological standpoint. The
relevance of diabetes at clinical and individual level is given by its lifethreatening
acute complications and, especially, by its chronic complications
affecting several organs and systems, with increased risk for ocular, renal,
cardiac, cerebral, nervous and peripheral vascular diseases.
Chemoprevention β-Carotene and cis-retinoic acid can lead to the regression of leukoplakia. However, cis-retinoic acid does not reduce the incidence of second primaries
Complications from treatment of head and neck cancer are usually correlated to the extent of surgery and exposure of normal tissue structures to radiation. Currently, the extent of surgery has been limited or completely replaced by chemotherapy and radiation therapy as the primary approach. Acute complications of radiation include mucositis and dysphagia.
Acute emergencies in gastroenterology are extraordinarily
severe conditions with high morbidity and mortality.
Particularly severe diseases include acute pancreatitis,
a difficult course of non-specific intestinal inflammations
manifested by toxic colon or acute intestinal obstruction,
and even acutely developed intestinal pseudo-obstruction
(Ogilvie’s syndrome) or variceal and non-variceal bleeding
into the gastrointestinal tract.
Diagnosis of acute pediatric sinusitis must be
made on the basis of signs, symptoms, and
clinical examination. Laboratory tests may be
normal even in complicated sinusitis (Pitkäranta
et al. 1999; Hytönen et al. 2000), and radiolog-
ical fi ndings do not necessarily correlate with a
child’s clinical condition. Pathological fi ndings
are frequently found in plain radiographs, CT,
and MRI of children with common colds, and
also in children without symptoms of a respi-
ratory infection (Kronemer and McAlister 1997;
American Academy of Pediatrics 2001; Schwartz
Cardiac arrest is cessation of heart action. Ventricular standstill (asystole) and ventricular fibrillation are the immediate causes, but the underlying etiologies are most frequently acute myocardial hypoxia or alteration in conduction or both. In obstetrics and gynecology, cardiac arrest occurs during induction of anesthesia and during operative surgery or instrumented delivery. Cardiovascular disease increases the risk of cardiac arrest, and hypoxia and hypertension are contributory causes.

Management of acute pain, especially post-operative pain, in patients on high dose opioids is a challenge
that requires in-depth knowledge of pharmacokinetics and the formulation of a careful management plan
to avoid withdrawal symptoms and inadequate pain management.
• Chronic pain after cancer surgery may occur in up to 50% of patients. Risk factors for the development of
chronic pain after breast cancer surgery include: young age, chemo and radiotherapy, poor post-operative
pain control and certain surgical factors.
