The committee is grateful for the contribution of participants at its three public meetings. The
presentations and discussions at these meeting were valuable in informing the committee about relevant
research findings, issues of interest in the research community, the perspectives of advocacy
organizations, and the concerns of individuals with breast cancer and their families. The agendas for these
meetings appear in Appendix A.

From May 1997 to October 2000 he was Executive Director of IMnI, an
international, metal mining consortium based in Paris. He was Chief Executive of
VSL (UK-based professional organisation specialising in innovation management
and investment, European/USA strategic alliances and international licensing) from
April 1993 to December 2000 and in addition he was Chairman of Royston Lead
plc from January 1996 to October 1997.

Experts were located in two ways. First, nearly a thousand were identified in an extensive canvassing of
scholarly, government, and business documents from the period 1990-1995 to see who had ventured
predictions about the future impact of the Internet. Several hundred of them participated in the first two
surveys conducted by Pew Internet and Elon University, and they were recontacted for this survey.
The increasing focus on international health is evident in several large U.S.A.
organizations. The International Health Medical Education Consortium (now called the
Global Health Education Consortium), created in 1991, now has a membership of
approximately 80 medical schools in the U.S.A. and Canada and aims to foster
international health education for medical students. The American Medical Association
opened its Office of International Medicine in 1978, the U.S.A.
Sample diets are shown in Tables 2 and 3 for the respective 5-8 and 17-19 wk periods for
Treatments 1 through 4. The control diet (Treatment 1) includes animal protein because of its
obvious economic advantage and widespread use. Valine content (as a percent of protein) is
similar across ingredients; therefore diet protein in these sample diets was fixed by setting a
valine specification. Supplemental lysine, methionine, and threonine were used so that all diets
contained adequate amounts of these amino acids.
