  • The Board’s Patient Care Assessment (PCA) program, receives three kinds of reports from hospitals: Major Incident Reports (MIR), detailing events resulting in death or serious impairment of a patient; and Annual and Semi-Annual Reports which detail a facility’s progress with respect to its patient safety program.
