  • Pancreatic cancer is one of the most fatal human malignancies with extremely poor prognosis making it the fourth leading cause of cancer-related deaths in the United States. The molecular mechanisms of pancreatic carcinogenesis are not well understood. The major focus of these two books is towards the understanding of the basic biology of pancreatic carcinogenesis, identification of newer molecular targets and the development of adjuvant and neoadjuvant therapies.

  • Cause-specific survival is commonly estimated in cancer clinical trials and only those deaths which can be attributed to the cancer in question are considered to be events, while all other deaths are considered censorings. Using cause-specific survival to estimate net survival requires that reliably coded information on cause of death is available. The distinguishing feature of survival analysis is that at the end of the follow-up period the event (such as death due to cancer) will probably not have occurred for all patients.

  • Many cancer patients are diagnosed at a stage in which the cancer is too far advanced to be cured, and most cancer treatments are effective in only a minority of patients undergoing therapy. Thus, there is tremendous opportunity to improve the outcome for people with cancer by enhancing detection and treatment approaches. Biomarkers will be instrumental in making that transition.

  • Antibodies In general, antibodies are not very effective at killing cancer cells. Because the tumor seems to influence the host toward making antibodies rather than generating cellular immunity, it is inferred that antibodies are easier for the tumor to fend off. Many patients can be shown to have serum antibodies directed at their tumors, but these do not appear to influence disease progression. However, the ability to grow very large quantities of high-affinity antibody directed at a tumor by the hybridoma technique has led to the application of antibodies to the treatment of cancer.

  • Clinical trials enable scientific discoveries to advance patient care, and they also inform and guide subsequent research. The National Cancer Institute (NCI) supports the largest U.S. network of clinical trials of any type, of which the largest component is the Clinical Trials Cooperative Group Program (informally known as the Cooperative Group Program). It currently comprises 10 Groups that involve more than 3,100 institutions and 14,000 investigators who enroll more than 25,000 patients in clinical trials each year.

  • A second potential source of the discrepancies may come from how the exposure or lack of exposure to environmental tobacco smoke is determined. Studies have shown that people can recall recent exposure very well but that remembering the duration and degree of distant exposure (such as whether their grandparents or baby-sitter smoked) is difficult. Yet one study examined this issue and found that women tended to underestimate their exposure, an effect which would decrease the observed risk.

  • Clearly a more stratified approach to clinical trials would help identify those subgroups who appear to be the best responders to a particular intervention. To date however there is little to suggest that stratification on the basis of clinical characteristics successfully helps predict which drugs work best for which patients. There is a pressing need for the development of biomarkers with clinical utility, for mental health problems.

  • Isolated blunt abdominal trauma (BAT) represents about 5% of annual trauma mortality from blunt trauma. As part of multiple-site injury (polytrauma), BAT contributes another 15% of trauma mortality. In the abdominal trauma, the best exploration strategy is one that leads most quickly and reliably in the diagnosis of surgical injury. This strategy must be established based on hemodynamic status and clinical guidance but should never delay a therapeutic homeostasis.

  • Neoplastic disease In most cases, the cause of cancer is multifactorial. About 75% of cancers are due to environmental factors, some of which are within the control of the individual, e.g. tobacco smoking, exposure to sunlight. Growing understanding of cancer genetics and inherited disease suggests that fewer than 10% of cancers are familial. The different systemic modalities used to treat cancer patients are discussed. Immunosuppressive drugs are described here as they share many characteristics with cytotoxics. ...
