  • Unpublished data from Brigham and Women's Hospital Chest Pain Study, 1997–1999 Markers of myocardial injury are often obtained in the emergency department evaluation of acute chest discomfort. The most commonly used markers are creatine kinase (CK), CK-MB, and the cardiac troponins (I and T). Rapid bedside assays of the cardiac troponins have been developed and shown to be sufficiently accurate to predict prognosis and guide management.

  • Pulmonary Embolism (See also Chap. 256) Chest pain due to pulmonary embolism is believed to be due to distention of the pulmonary artery or infarction of a segment of the lung adjacent to the pleura. Massive pulmonary emboli may lead to substernal pain that is suggestive of acute myocardial infarction. More commonly, smaller emboli lead to focal pulmonary infarctions that cause pain that is lateral and pleuritic. Associated symptoms include dyspnea and, occasionally, hemoptysis. Tachycardia is usually present.

  • Angina Pectoris (See also Chap. 237) The chest discomfort of myocardial ischemia is a visceral discomfort that is usually described as a heaviness, pressure, or squeezing (Table 13-2). Other common adjectives for anginal pain are burning and aching. Some patients deny any "pain" but may admit to dyspnea or a vague sense of anxiety. The word "sharp" is sometimes used by patients to describe intensity rather than quality.

  • Pulmonary embolism is a serious, potentially life-threatening cardiopulmonary disease that occurs due to partial or total obstruction of the pulmonary arterial bed. Pulmonary embolism constitutes 5-25% of in-hospital deaths, and mortality is decreased from 30% to 8% with early treatment. Therefore, risk factors should be identified and treatment should be planned to decrease the risk of mortality. Clinical findings, routine laboratory data, electrocardiogram, chest X-ray, and arterial blood gases are not sufficient to diagnose or rule out pulmonary embolus.

  • EventReporter is an integrated, modular and distributed solution for system management. Microsoft Windows NT™, Windows 2000™ and Windows XP™ are highly capable operating systems (we will call all of them "NT" in the following documentation). However, their standard event reporting mechanisms are rather limited. Administrators seeking complete control over their server environment need to regularly check the server event logs. Adiscon's EventReporter provides central notification of any events logged to the NT system event log.

  • First published in 1992, Clinical Problems in General Medicine and Surgery was principally written for senior medical students and interns, about to or already experiencing the real world of medical practice. It was a departure from 'traditional' medical texts in that the book dealt with the real problems of clinical medicine through scenarios encountered in the emergency department, on the wards and in outpatient clinics.

  • In this paper we consider the following problem. Given a class of episodes and an input sequence of events, find all episodes that occur frequently in the event sequence. We describe the framework and formalize the discovery task in Section 2. Algorithms for discovering all frequent episodes are given in Section 3. They are based on the idea of first finding small frequent episodes, and then progressively looking for larger frequent episodes. Additionally, the algorithms use some simple pattern matching ideas to speed up the recognition of occurrences of single episodes.
