About three-quarters of people with schizophrenia suffer recurrent relapses and continued
disability. Outcome appears to be worse in people with the following factors: 1) an insidious
onset of symptoms where initial treatment is delayed; 2) social isolation; 3) a strong family
history of schizophrenia or other major mental disorder; 4) people living in industrialised
countries and urbanised communities; 5) men appear to fair worse than women; and 6) in
those people who misuse drugs especially cannabis, and possibly from an early age under 16
years (Jablensky et al 1992).
With the proliferation of Web and multimedia services,
and virtual private networks (VPNs) connecting corporate
sites, more versatile Internet routing protocols have become
critical. Current Internet packet forwarding is based
on the destination address. Simple routing algorithms that
determine forwarding paths based on the minimum number
of hops or delay to a specified destination are no longer
sufficient. The need to support diverse traffic types and
applications with quality demands (see Figure 1) imposes
new requirements on routing in the (now commercial) Internet.

The primary remaining problemin keymanagement is a technical issue; namely
how to locate the certiﬁcate for a certain machine or person.
Some basic requirements on the facility used to locate the certiﬁcate can easily
be identiﬁed. It must be accessible everywhere. It should be practical, in the sense
that it should not be extremely expensive or cause administrative hassle, to work.
Preferably it should be distributed, because a central world-wide organization to
store all certiﬁcates is not feasible to implement.
More specifically, the recommended assessment should identify the occurrence (in
space and time) of both surface and groundwater quantity and their associated water
quality, together with a tentative assessment of trends in water requirements and water
resources development (see Box 10.1). The assessment should be based, as far as
possible, on existing data and knowledge in order to avoid unnecessary delays in the
process of management improvement.

Vast majority of active tuberculosis patients seeks treatment,
do so promptly, still many patients spend a great deal of time
and money “shopping for health” and too often they do not receive
either accurate diagnosis or effective treatment, despite
spending considerable resources. Objective: To find out the
time taken to, for diagnosis of tuberculosis and to put patient
on DOTS from the onset of symptoms and pattern of health
seeking behavior of new pulmonary tuberculosis patients.
