The differential diagnostic possibilities are much fewer when the spleen is "massively enlarged," palpable more than 8 cm below the left costal margin or its drained weight is ≥1000 g (Table 60-3). The vast majority of such patients will have non-Hodgkin's lymphoma, chronic lymphocytic leukemia, hairy cell leukemia, chronic myelogenous leukemia, myelofibrosis with myeloid metaplasia, or polycythemia vera.
Many psychiatric institutions, general hospitals and social care homes in countries continue to use caged beds routinely
to restrain patients with mental disorders and mental retardation. Caged beds are beds with netting or, in some cases,
metal bars, which serve to physically restrain the patients. Patients are often kept in caged beds for extended periods,
sometimes even years. This type of restraint is often used when staff levels or training are inadequate, and sometimes
as a form of punishment or threat of punishment.
Dendritic cell-specific intracellular adhesion molecule-3-grabbing noninte-grin (DC-SIGN) is a member of the C-type lectin family selectively
expressed on immune-related cells. In the present study, we performed
a systematic interaction analysis of DC-SIGN and its related receptors,
DC-SIGN-related protein (DC-SIGNR) and liver and lymph node sinusoi-dal endothelial cell C-type lectin (LSECtin) using frontal affinity chroma-tography (FAC).
An extended range of new sensing applications and solutions have emerged in different fields such
as rural and forest fire detection , monitoring of industrial manufacturing processes ,
transport/logistics , and for personal services in areas such as healthcare and Ambient Assisted
Living (AAL) . These new advanced applications based on a new generation of sensing and

and wireless communication protocols that allow for greater autonomy of sensor nodes.

Sensors have been used in both civil and military applications, in order to extend the ﬁeld of view of the end-user. However, most current sensing systems consist of a few large macrosensors, which while being highly accurate are expensive. Macrosensor systems are highly sensitive; the entire system can break down even with one faulty sensor.

This paper describes an unsupervised dynamic graphical model for morphological segmentation and bilingual morpheme alignment for statistical machine translation. The model extends Hidden Semi-Markov chain models by using factored output nodes and special structures for its conditional probability distributions. It relies on morpho-syntactic and lexical source-side information (part-of-speech, morphological segmentation) while learning a morpheme segmentation over the target language. Gastric Adenocarcinoma: Treatment Complete surgical removal of the tumor with resection of adjacent lymph nodes offers the only chance for cure. However, this is possible in less than a third of patients. A subtotal gastrectomy is the treatment of choice for patients with distal carcinomas, while total or near-total gastrectomies are required for more proximal tumors. The inclusion of extended lymph node dissection in these procedures appears to confer an added risk for complications without enhancing survival.
Lymphomas Lymphoma in the small bowel may be primary or secondary. A diagnosis of a primary intestinal lymphoma requires histologic confirmation in a clinical setting in which palpable adenopathy and hepatosplenomegaly are absent and no evidence of lymphoma is seen on chest radiograph, CT scan, or peripheral blood smear or on bone marrow aspiration and biopsy. Symptoms referable to the small bowel are present, usually accompanied by an anatomically discernible lesion.
