This book deals with problems which are sufficiently important to become the subject of studies.
Cancer of the stomach remains one of the most pressing medical problems. Meanwhile,
scientific and practical interest in this problem has markedly diminished during recent years.
According to some experts, this can be explained first by the decreasing incidence of gastric
cancer. But this concerns only some developed countries, where effective measures are taken
for the prevention and early diagnosis of malignant tumors.
Previous studies have shown an anticancer effect of vitamin D, but the
mechanisms underlying this action have not been fully explored. Here we
show that 1,25-dihydroxyvitamin D3 (VD3, the active form of vitamin D)
significantly promoted apoptosis in the undifferentiated gastric cancer cell
line HGC-27, and this was accompanied by a concurrent increase in phos-phatase and tensin homolog deleted on chromosome 10 (PTEN) expression
on VD3 treatment.
Helicobacter pylori is a Gram-negative bacterium that has been associated with diverse pathologies of varying severity, such as chronic gastritis, peptic ulcer, mucosa-associated lymphoid tissue (MALT) lymphoma, and gastric carcinoma.1 Although most infected persons remain asymptomatic, 15–20% of H. pyloripositive individuals will develop the associated diseases. This could be dependent on environmental factors, host genetic factors or specific properties of the microorganism.1
Less than 3 decades ago, Robin Warren and Barry Marshall definitively identified Helicobacter pylori by culturing an organism from gastric biopsy specimens that had been visualized for almost a century by pathologists (196). In 1994, H. pylori was recognized as a type I carcinogen, and now it is considered the most common etiologic agent of infection-related cancers, which represent 5.5% of the global cancer burden (239). In 2005, Marshall and Warren were awarded the Nobel Prize of Medicine for their seminal discovery of this bacterium and its role in peptic ulcer disease

Incidence and Epidemiology
For unclear reasons, the incidence and mortality rates for gastric cancer have decreased markedly during the past 75 years. The mortality rate from gastric cancer in the United States has dropped in men from 28 to 5.8 per 100,000 persons, while in women the rate has decreased from 27 to 2.8 per 100,000. Nonetheless, 21,260 new cases of stomach cancer were diagnosed in the United
States, and 11,210 Americans died of the disease in 2007.
