  • Harrison’s Principles of Internal Medicine has a long and dis- tinguished tradition in the field of hematology. Maxwell Wintrobe, whose work actually established hematology as a distinct subspecialty of medicine, was a founding editor of the book and participated in the first seven edi- tions, taking over for Tinsley Harrison as editor-in-chief on the sixth and seventh editions. Wintrobe, born in 1901, began his study of blood in earnest in 1927 as an assistant in medicine at Tulane University in New Orleans.

  • Incidence and Prevalence About 19,900 cases of myeloma were diagnosed in 2007, and 10,790 people died from the disease in the United States. Myeloma increases in incidence with age. The median age at diagnosis is 68 years; it is uncommon under age 40. The yearly incidence is around 4 per 100,000 and remarkably similar throughout the world. Males are more commonly affected than females, and blacks have nearly twice the incidence of whites. Myeloma accounts for ~1% of all malignancies in whites and 2% in blacks; 13% of all hematologic cancers in whites and 33% in blacks. The incidence of...

  • Hematologic Syndromes: Introduction The elevation of granulocyte, platelet, and eosinophil counts in most patients with myeloproliferative disorders is caused by the proliferation of the myeloid elements due to the underlying disease rather than a paraneoplastic syndrome. The paraneoplastic hematologic syndromes in patients with solid tumors are less well characterized than the endocrine syndromes because the ectopic hormone(s) or cytokines responsible have not been identified in most of these tumors (Table 96-2).

  • The vast majority of published cytogenetic studies of malignancy have been of leukemias and related hematologic disorders (see Fig. 1), even though these constitute only about 20% of all cancers. It follows that most of what is known about the clinical applications of cytogenetic studies has been derived from hematologic malignancies.

  • So ended the editorial entitled “Some Speculations on the Myeloproliferative Syndomes” published in Blood in 1951 by the journal editor, William Dameshek. He speculated that these various conditions, which he had termed “myeloproliferative,” were all somewhat variable manifestations of proliferative activity of the bone marrow cells, perhaps due to “a hitherto undiscovered stimulus.

  • Epidemiological studies conducted during the past decade revealed that CD is one of the most common lifelong disorders worldwide. CD can manifest with a previously unsuspected range of clinical presentations, including the typical malabsorption syndrome and a spectrum of symptoms potentially affecting any organ system. Since CD is often atypical or even silent on clinical ground, many cases remain undiagnosed and exposed to the risk of long term complications, such as anemia and other hematological complications, osteoporosis, neurological complications or cancer....

All of these individuals will, at some time in their professional lives, be faced with quantitative results (their own or those of others) that will need to be critically evaluated and interpreted, and some, of course, will have to pass that dreaded statistics exam! A proper understanding of statistical concepts and methodology is invaluable for these needs.

  • The medical devices industry is booming. Growth in the industry has not stopped despite globally fluctuating economies. The main reason for this success is probably the self-sustaining nature of health care. In essence, the same technology that makes it possible for people to live longer engenders the need for more health-care technologies to enhance the quality of an extended lifetime. It comes as no surprise, then, that the demand for trained medical-device designers has increased tremendously over the past few years.

  • The negative health impact of many large-scale public health emergencies, such as an intentional anthrax release, severe acute respiratory syndrome (SARS), or the recent 2009 H1N1 influenza pandemic, can be mitigated significantly by medical countermeasures such as antimicrobials, antivirals, and vaccines. To be effective, these countermeasures generally must be delivered in very large quantities in a short period of time.

  • Primary care clinicians often provide continuity of care, emphasize prevention, and address chronic illness with their patients. All these endeavors require the active involvement of the patient in treatment planning and adherence. An active and healthy therapeutic alliance that recognizes that patients are the chief consumers of health care is essential in this regard. This critical role in directing one’s health care can be either facilitated or complicated by a patient’s emotional, cognitive, and behavioral strengths or weaknesses (ie, mental health).
