  • Despite significant accomplishments to date, kidney transplantation is a relatively young field in medicine. Due to the armamentarium of agents available to effectively suppress the immune system, the past decade has seen a shift in focus from prevention of rejection to a focus on extending the life of the allograft and novel strategies to increase the organ donor pool. This book covers basic concepts in kidney transplantation while also addressing ways to manage kidney transplant recipients in order to maximize patient and graft survival.

  • Your kidneys filter wastes from your blood and regulate other functions of your body. When your kidneys fail, you need treatment to replace the work of healthy kidneys to survive. Developing kidney failure means that you have some decisions to make about your treatment. If you choose to receive treatment, your choices are hemodialysis, peritoneal dialysis, and kidney transplantation. Each of them has advantages and disadvantages. You may also choose to forgo treatment.

  • Middle-Period Infections Because of continuing immunosuppression, kidney transplant recipients are predisposed to lung infections characteristic of those in patients with T cell deficiency (i.e., infections with intracellular bacteria, mycobacteria, nocardiae, fungi, viruses, and parasites). The high mortality rates associated with Legionella pneumophila infection (Chap. 141) led to the closing of renal transplant units in hospitals with endemic legionellosis.

  • Kidney transplant recipients are also subject to infections with other intracellular organisms. These patients may develop pulmonary infections with Nocardia, Aspergillus, and Mucor as well as infections with other pathogens in which the T cell/macrophage axis plays an important role. In patients without IV catheters, L. monocytogenes is a common cause of bacteremia ≥1 month after renal transplantation and should be seriously considered in renal transplant recipients presenting with fever and headache.

  • Developing kidney failure means you have some decisions to make about your treatment. You may choose to forgo treatment. If you choose to receive treatment, your choices include hemodialysis, which requires a machine used to filter your blood outside your body; peritoneal dialysis, which uses the lining of your belly to filter your blood inside the body; and kidney transplantation, in which a new kidney is placed in your body. Each treatment has advantages and disadvantages.

  • The third section of this book is ‘Adeno-associated-virus Vector’. Chapter nine by Sun et al. introduces the adeno-associated virus (AAV) mediated β-thalassemia gene therapy. Human hematopoietic stem cells (HSCs) were obtained from β-thalassemia patients, transfected with the recombinant AAV containing β-globin gene. The transfected cells were then transplanted into Nude/SCID mice, and the long term expression of β-globin in vivo was examined. In the tenth chapter, Korecka et al. compare AAV serotypes for gene delivery to dopaminergic neurons in the substantia nigra (SN)....

  • The field of solid organ transplantation has developed enormously in the last three decades andwhat was pioneering surgery has now become routine. Outcomes are no longer considered in terms of 1-year survival, but clinicians and patients are looking to 20 years and beyond. The current success of transplantation is based on many different factors: developments in surgical technique, better immunosuppression, improved anesthetic and intensive care, improved microbiology, and close collaboration between – all those involved in the transplant pathway have contributed....

  • I have been involved in the treatment of chronic renal insuffi ciency for 40 years, beginning with peritoneal dialysis immediately after graduation from medical school in 1965, then with hemodialysis in 1967 after I fi rst experienced it in Kanazawa, and with renal transplantation since 1972, when I was studying in the United States. During this period, the number of dialysis patients has continued to increase rapidly to the present fi gure of 257 765 (at the end of 2005), and with surprising increases in the survival rate.
