  • The spleen is the intra-abdominal organ most often injured as a result of blunt trauma. The spleen is the most vascular organ of the body and, for this reason, splenic injury is potentially life threatening. The most common mechanism for such injury is motor vehicle collision. Left lower rib fractures are suggestive of spleen injury, although an intact rib cage does not exclude spleen trauma.

  • We located one study in health education that compared attention given to just text with attention given to text plus pictures. This study, by Delp and Jones [2], studied 234 patients coming to an emergency room with lacerations. After receiving treatment, but prior to discharge, patients were given printed instructions for caring for their wounds at home. Half were randomly given just text and the other half received the same text plus pictures that illustrated the information in the text. Fig. 1 shows examples of the text and pictures they used.

  • It is common practice to recommend nutritional supplementation, such as multi-vitamins and iron, following bariatric surgery for all patients. However, evidence is scarce regarding specific recommendations for pregnant women. We did not identify any randomized studies assessing this question of efficacy of management strategies. Studies evaluating pregnancy following laparoscopic adjustable gastric banding or gastric bypass have shown minimal nutritional adverse events; however, most of these studies monitored and ensured that the women complied with vitamin supplementation.

  • Renal Cell Carcinoma: Treatment Localized Tumors The standard management for stage I or II tumors and selected cases of stage III disease is radical nephrectomy. This procedure involves en bloc removal of Gerota's fascia and its contents, including the kidney, the ipsilateral adrenal gland, and adjacent hilar lymph nodes. The role of a regional lymphadenectomy is controversial. Extension into the renal vein or inferior vena cava (stage III disease) does not preclude resection even if cardiopulmonary bypass is required.
