Patients provide valid informed consent to a treatment or a
diagnostic procedure if they have sufficient capacity, have been
given appropriate information, and give consent freely without coercion
or undue influence. When a patient’s capacity for treatment
consent is in doubt, a clinician must determine whether the patient
indeed has the capacity. It is a common reason behind requests for
psychiatric consultations in a general hospital (Appelbaum, 2007).

The patient admitted to the hospital is already an individual who is at risk because
of an underlying pathology that requires inpatient care. When patients decline,
they have evidence of clinical deterioration (hypoxia, hypotension, tachypnoea,
tachycardia, altered level of consciousness) that is often documented in the medical
record 8 to 48 hours prior to a crisis being detected
Information on long-term health consequences of unsafe abortion is
scarce. The WHO estimates that about 20–30% of unsafe abortions
result in reproductive tract infections and that about 20–40% of these
result in upper-genital-tract infection and infertility. An estimated 2% of
women of reproductive age are infertile as a result of unsafe abortion,
and 5% have chronic infections.
Unsafe abortion could also increase
the long-term risk of ectopic pregnancy, premature delivery, and
spontaneous abortion in subsequent pregnancies.

A little over 50 years ago, my father had a heart
attack. He was driven to the hospital by friends
after having “indigestion” for 2 days. He spent
2 weeks as an inpatient on an unmonitored rehabilitation
ward and was treated principally with
warfarin and digitalis. He was lucky and survived,
but in that era, more than 20% of patients
with an acute myocardial infarction died.

Some medical schools have a dedicated ambulatory internal medicine rotation,
others combine this experience with the inpatient rotation, and some
hybridize it with other primary care fields. Often thought of as a less essential
and less exciting rotation, let it be said that the outpatient encounter is where
a lot of action happens. Keeping people well, establishing long-term relationships,
and having the privilege of bearing witness to improving health or even
declining health are, at least to me, the essence of what it means to not just be
a doctor but someone’s doctor.
