  • Thyroid and parathyroid diseases are among the most frequent conditions we have to deal with in a clinical setting. Both nodular and autoimmune diseases affecting these glands have increased remarkably over the past decades. The widespread use of crosssectional imaging and the introduction of neck ultrasonography have led to a thyroid nodule epidemics and the diagnostic of neck lesions in half of the population. Consequently, many patients with microscopic papillary thyroid cancers of uncertain clinical significance are submitted to surgeries that may, perhaps, be unnecessary....

  • Recurrent CDAD Overall, ~15–30% of patients experience recurrences of CDAD, either as relapses caused by the original organism or as reinfections following treatment (Table 123-2). Recurrence rates are higher among patients ≥65 years old and among patients who remain in the hospital after the initial episode of CDAD. Patients who have a first recurrence of CDAD have a high rate of second recurrence (33–65%). In the first recurrence, re-treatment with metronidazole is comparable to treatment with vancomycin.

  • It was November 1901, and Karl Deter of Germany did not know what to do about his wife, Auguste. Over the previous several months, her behavior had changed drastically. Only fiftyone years old, Auguste had lost the ability to remember things from one minute to the next. She had lost the ability to express herself with words. She was often confused about what day it was, where she was, and even who she was. She had frequent outbursts of anger and paranoid thinking. She would hide things, thinking her family was trying to steal from her.

  • The belief that children will take care of the parents in the old age is eroding in India where the family size has been cut down as a result of the demographic process (Dandelkar, 1996). The situation in the urban areas shows a rejection of older people by the next generation and this is spreading to rural areas (Desai, 1985). In the nuclear family regime, the position of the aged becomes more vulnerable and is treated as a burden to the family (Nayar, 1992). The social negligence of the aged occurs due to cultural, social and economic relations within the...

  • The old saying, “You can’t get fired for hiring IBM” just isn’t valid anymore. These days, clients choose the best consultants, not the best-known ones. Today’s clients seek talent, not firm names. The competition for new work is not between firms, but between people and their ideas. Consultants’ marketing efforts haven’t changed in response to this reality. In fact, their marketing hasn’t changed much in decades—except to get slicker, flashier and more expensive. That’s not working.

  • While this and the other Workman montages ultimately elude any causally or narratively-based rhetorical analysis as persuasive marketing devices, their very anti- causality within a broadly thematic framework is of interest in terms of what is and isn’t valorized by these montages as constituting “the movies.” The montage’s argument could be summed up as: “Movies: there’s old ones, new ones, lots of kinds, lots of emotions, there are heroes and the women they fight for, and most of all, there’s love and light.

  • Dystonia has many facets, and among those, this book commences with the increasingly associated genes identified, including a construct on how biology interacts with the dystonia genesis. The clinical phenomenology of dystonia as approached in the book is interesting because, not only were the cervical, oromandibular/lingual/laryngeal, task-specific and secondary dystonias dealt with individually, but that the associated features such as parkinsonism, tremors and spasticity were also separately presented.

Microscopically, the smears of this patient showed both intracellular and extracellular black pigment with obscured cytomorphology; although, appreciable cells were reported to be pleomorphic with prominent nucleoli. • But also important is how a recovery is measured, and how success is defined. The recent op-eds focus on GDP growth immediately following the trough (usually 4 quarters). For a normal recession, the restoration of positive growth is typically a signal event. In a v-shaped recovery, the old peak level of GDP is quickly reached, and the economy returns to trend within a year or two. In Reinhart and Rogoff (2009), we examine both levels and rates of change of per capita GDP; recovery is defined by the time it takes for per capita GDP...

  • However, in State v. Jones, 151 S.W.3d 494 (Tenn. 2004), the court ruled that a passenger’s conduct in holding her 2-year-old child in her lap was not a gross deviation from ordinary care under the facts of this case. The court reasoned that adult drivers routinely fail to safely restrain child passengers. The court cited a recent survey finding that only 60 percent of child passengers were restrained and that State law “permitted a mother to remove her child from its car seat to nurse the child or to ‘attend to its other physiological needs.’” 11 Likewise, in...
