  • This article introduces quasi-Darwinian selection as a new explanatory paradigm for marketing relationships. In this paradigm, established relationships are viewed as survivors of a selection process whose parameters are the conduct of the partners, dependencies between the partners, and external adversities in the markets. Selection has the effect of culling certain combinations of these parameters, such as attempts at unilateral control when the partner is not dependent.

Patients were divided into five categories on the basis of symptoms at presentation (none, nonanginal chest pain, atypical angina, typical angina, and dyspnea).

  • The importance of the anesthetist in perioperative care cannot be too greatly emphasized. Correct patient selection and procedure planning can only be optimized by a team approach and together with the surgeon; the anesthetist forms the core of the team. A thorough understanding of the underlying physiology of the gastrointestinal tract is important and a logical starting place for this book.

  • The Acutely Ill Patient: Treatment In the acutely ill patient, empirical antibiotic therapy is critical and should be administered without undue delay. Increased prevalence of antibiotic resistance in community-acquired bacteria must be considered when antibiotics are selected. Table 115-1 lists first-line treatments for infections considered in this chapter. In addition to the rapid initiation of antibiotic therapy, several of these infections require urgent surgical attention.

  • In neurointensive patients with acute lung injury, achieving the goal of lung protection without threatening cerebral perfusion is very difficult. In patients with more refractory raised intracranial pressure, the optimal balance between brain and lung may not be well established. Multiple strategies are used to recruit atelectatic alveoli and improve oxygenation. Among them incremental levels of PEEP and high intermittent tidal volumes should require extend brain physiological monitoring.

  • Factors predisposing to colorectal cancer in UC patients consist of pancolitis, duration of disease, active disease and its severity. Early UC onset is another independent risk factor. Beside these factors, dysplasia represents the precancerous lesion from which colorectal cancer subsequently arise (Morson, 1962; Morson & Pang, 1967). In fact, 70% of patients with colorectal cancer on UC have dysplasia on colorectal mucosa (Taylor et al., 1992; Connell et al., 1994).

  • PDi and LDi are aimed at testing the incidence of adverse selection: whether firms in poor financial health and/or facing liquidity constraints are more likely to seek and get access to bank credit. In the case of the liquidity dummy there is no ambiguity about the causality and the interpretation of the results in terms of adverse selection. However, in the case of the profitability dummy, again we cannot fully eliminate the endogeneity problem because – as mentioned before – firm’s profit/loss position may affect also bank’s decision to extend the loan.

  • Soft tissues and exudates are best evaluated using contrast medium, therefore the best imaging technique is contrast CT-scan. The soft tissues, spaces and exudates appear radioopaque on contrast CT scans. Moreover, CT scan is cheaper, readily available and has no electromagnetic effects on patients with metallic implants compared to MRI and most patients do not react to the contrast medium (Gadolinium) which is injected into the body via intravenous route before the scan.
