  • Microvascular perfusion is often impaired after primary percutaneous coronary intervention (PCI). We proposed that in situ thrombosis might contribute to poor myocardial perfusion in this setting. To test this hypothesis, we evaluated the effect of lowdose intracoronary streptokinase administered immediately after primary PCI. Methods Forty-one patients undergoing primary PCI were randomly assigned to receive intracoronary streptokinase (250 kU) or no additional therapy.

  • Despite its limitations, unfractionated heparin has been the standard anticoagulant used during percutaneous coronary intervention (PCI). Several small studies have suggested that intravenous enoxaparin may be a safe and effective alternative. Our primary aim was to assess the safety of enoxaparin as compared with that of unfractionated heparin in elective PCI. Methods In this prospective, open-label, multicenter, randomized trial, we randomly assigned 3528 patients with PCI to receive enoxaparin (0.5 or 0.

  • The perfect cardiac catheterization technique, including good diagnostic and therapeutic qualities, without risk and with no recovery time for the patient, does not exist. Obtaining initial access to the arterial circulation is the first and most frequent catheterization difficulty encountered by the interventional cardiologist during the procedure. Often, it is also the only difficult part of the exam for the patient because it may cause a vagal reaction or painful spasm.

  • It is only 26 years since the first percutaneous transluminal coronary angioplasty (PTCA) was carried out by the pioneering Swiss radiologist, Andreas Greuntzig, heralding the dawn of interventional cardiology. In this short time, interventional cardiology has overcome many limitations and undergone major evolutionary changes—most notably the development of the coronary stent. Worldwide, many thousands of patients now safely undergo percutaneous coronary intervention every day, and the numbers continue to grow.

  • Several studies have compared outcomes for coronary-artery bypass grafting (CABG) and percutaneous coronary intervention (PCI), but most were done before the availability of stenting, which has revolutionized the latter approach. methods We used New York’s cardiac registries to identify 37,212 patients with multivessel disease who underwent CABG and 22,102 patients with multivessel disease who underwent PCI from January 1, 1997, to December 31, 2000.

  • In patients with stable coronary artery disease, it remains unclear whether an initial management strategy of percutaneous coronary intervention (PCI) with intensive pharmacologic therapy and lifestyle intervention (optimal medical therapy) is superior to optimal medical therapy alone in reducing the risk of cardiovascular events. Methods We conducted a randomized trial involving 2287 patients who had objective evidence of myocardial ischemia and significant coronary artery disease at 50 U.S. and Canadian centers....

  • We have been able to develop 23 Regional Cancer Centres and several Oncology Wings in India, which provide comprehensive cancer care services. One of the major limitations of the programme is the late stage at presentation of common cancers thus reducing the chances of survival. There is a need to increase awareness among the community regarding prevention and early detection of cancers. The programme is developing IEC materials for the same.

  • WHO has a long tradition in synthesizing the evidence on health aspects of air quality and in preparing air quality guidelines defining conditions for healthy air. We are grateful to the outstanding scientists conducting this work. We hope that these new guidelines will be useful globally to people assessing indoor air quality with a view to predicting its effects on health, and also to those with responsibil- ity for introducing measures to reduce health risks from indoor exposure to air pollutants.

  • The justification for choosing this typing scheme is scalability. In other sys- tems, such as one finds for example in the Java Distributed Event Specifica- tion [SunMicrosystems 1998] andCORBANotification Service [ObjectManage- ment Group 1998b], a notification is a value of some named, explicit notification type. This implies a global authority formanaging and verifying the type space, something which is clearly not feasible at an Internet scale. On the other hand, we define a restricted set of attribute types from which to construct (arbitrary) notifications.

  • In October 2005, an Angelina County, Texas, court held a woman accountable in an automobile crash that resulted in the death of her 1-month-old son who was not properly restrained in the vehicle. The woman forgot the child’s car seat and let a friend hold the child in her lap while a 14-year-old, unlicensed, uninsured driver drove the vehicle. The car slammed into an oncoming pickup truck and rolled over, killing the boy. The woman pleaded guilty to negligent homicide charges and was sentenced to a deferred three-year prison sentence and is required to attend...

  • The procedure described by Young involved a transverse incision between the ischial tuberosities 2 cm above the anus (Young, 1926). The ischiorectal fossa was then opened by blunt finger dissection, carrying the dissection medially and reflecting the rectum posteriorly. The central tendon of the perineum was then cut exposing the recto-urethralis muscle which was opened by sharp dissection and reflected laterally, exposing the capsule of the prostate (figure 5). The abnormal area on the prostate was grasped with Allis forceps and excised widely and deeply.

  • Health insurance makes a difference in whether and when people get necessary medical care, where they get their care, and ultimately, how healthy people are. Uninsured adults are far more likely than the insured to postpone or forgo health care altogether and less able to afford prescription drugs or follow through with recommended treatments. Problems getting needed care are less common among children, who are generally healthy, but disparities in access to care between uninsured and insured children are as great as the adult differences.

  • A range of factors contributed to this situation, such as the lack of access to basic health facilities - only 40% of the population is in the coverage areas of basic health facilities, and only 9% of rural households surveyed in 2003 reported a health facility in their village;1 lack of female staff at the existing facilities particularly in rural areas; marked rural-urban disparities in availability of health facilities; and lack of infrastructure (roads and transport) and security that reduce mobility and access.
