A range of policy options are available to address issues of high prices and low availability of medicines. Low public sector availability can be addressed through improved procurement efficiency, and adequate, equitable and sustainable financing.   • Exact evidence about incidence of sleep disorders in patients with epilepsy is not available, but it is supposed, that the amount is higher in patients with epilepsy than in general population. For example patients with partial epilepsy have twice higher appearence of sleep disorders as in healthy group (39% vs 18%) (Bazil, 2003). According to this study higher presence of sleep disorders did not correlate with antiepileptic therapy. According to Foldvary-Schaefer (2002) patients with epilepsy have problems with initialization of sleep and have worse quality of sleep.

Generalized epileptic seizures reduce total sleep time and elongate latency to REM sleep. Amount of NREM1 and NREM2 stages may be extended (Foldvary-Schaefer, 2002). Partial epileptic seizures during sleep do not disrupt night sleep markedly, only in case of their secondary generalization (Dasheiff, 2003).   • Unstable Angina and Myocardial Infarction (See also Chaps. 238 and 239) Patients with these acute ischemic syndromes usually complain of symptoms similar in quality to angina pectoris, but more prolonged and severe. The onset of these syndromes may occur with the patient at rest, or awakened from sleep, and sublingual nitroglycerin may lead to transient or no relief. Accompanying symptoms may include diaphoresis, dyspnea, nausea, and light-headedness. The physical examination may be completely normal in patients with chest discomfort due to ischemic heart disease.
