Recent experience illustrates this point. Consider the fact
that the Fed cut interest rates sharply in response to two of the
most serious financial crises in recent years: the October 1987
stock market break and the turmoil following the Russian
default in 1998. Arguably, in retrospect, interest rate policy
remained too easy for too long in both cases.
Convertible bonds can be difficult to value, given their hybrid nature of containing elements of both debt and equity.
Further complications arise due to the frequent presence of additional options such as callability and puttability,
and contractual complexities such as trigger prices and “soft call” provisions, in which the ability of the issuing firm
to exercise its option to call is dependent upon the history of its stock price.

Association rules represent a promising technique to find
hidden patterns in a medical data set. The main issue about
mining association rules in a medical data set is the large
number of rules that are discovered, most of which are irrelevant.
Such number of rules makes search slow and interpretation
by the domain expert difficult. In this work, search
constraints are introduced to find only medically significant
association rules and make search more efficient.
Control measures for pneumonia (Table 125-2) are aimed at the remediation of risk factors in general patient care (e.g., minimizing aspirationprone supine positioning) and at meticulous aseptic care of respirator equipment (e.g., disinfecting or sterilizing all inline reusable components such as nebulizers, replacing tubing circuits at intervals of 48 h—rather than more frequently—to lessen the number of breaks in the system, and teaching aseptic technique for suctioning).
Urinary Tract Infections Urinary tract infections (UTIs) account for as many as 40–45% of nosocomial infections; up to 3% of bacteriuric patients develop bacteremia. Although UTIs contribute only 10–15% to prolongation of hospital stay and to extra costs, these infections are important reservoirs and sources for spread of antibiotic-resistant bacteria in hospitals. Almost all nosocomial UTIs are associated with preceding instrumentation or indwelling bladder catheters, which create a 3–10% risk of infection each day.
Patients colonized with C. difficile were initially thought to be at high risk for CDAD. However, four prospective studies have shown that colonized patients actually have a decreased risk of subsequent CDAD. At least three events are proposed as essential for the development of CDAD (Fig. 123-2). Exposure to antimicrobial agents is the first event and establishes susceptibility to C. difficile infection. The second event is exposure to toxigenic C. difficile.
Recurrent CDAD Overall, ~15–30% of patients experience recurrences of CDAD, either as relapses caused by the original organism or as reinfections following treatment (Table 123-2). Recurrence rates are higher among patients ≥65 years old and among patients who remain in the hospital after the initial episode of CDAD. Patients who have a first recurrence of CDAD have a high rate of second recurrence (33–65%). In the first recurrence, re-treatment with metronidazole is comparable to treatment with vancomycin.

Low-carbohydrate diets have been advocated for weight loss and to prevent obesity,
but the long-term safety of these diets has not been determined.
We evaluated data on 82,802 women in the Nurses’ Health Study who had completed
a validated food-frequency questionnaire. Data from the questionnaire were
used to calculate a low-carbohydrate-diet score, which was based on the percentage
of energy as carbohydrate, fat, and protein (a higher score reflects a higher intake
of fat and protein and a lower intake of carbohydrate).
At the lower end of the uterus is the cervix, a narrow opening. It is
through the cervix that the sperm traveled on their way to the egg cell.
Connecting the cervix to the outside of the body is the vagina. The
vagina is made up of muscular tissue. It is into the vagina that sperm are
ejaculated from the penis. Also, the vagina is the birth canal, through
which the infant passes as it leaves the mother’s body during childbirth.
Unlike in males, the reproductive pathway in females is not combined
with the pathway for excretion.
