This book is a revised and updated edition of Asking Questions: A
Practical Guide to Questionnaire Design, first published in 1982. It
focuses on the type of question asking that social science researchers
and market researchers use in structured questionnaires or interviews.
Many of the principles of effective formalized questioning we
focus on in this book are useful in other contexts. They are useful
in informal or semistructured interviews, in administering printed
questionnaires in testing rooms, and in experimental studies involving
participant evaluations or responses....

Approach to the Patient: Chest Discomfort
The evaluation of the patient with chest discomfort must accommodate two goals—determining the diagnosis and assessing the safety of the immediate management plan. The latter issue is often dominant when the patient has acute chest discomfort, such as patients seen in the emergency department.
But all these allusions to what was said or what has happened are quite
unnecessary. The logic of the reform has an inexorability all its own: once
the reform is entrenched in K-12, the university courses would have no place
to go but to follow suit. Another induction step would result in a demand
for reform in the graduate program. Thus in no time at all, the burning
question of the day will be: are proofs allowed only in graduate courses?
From a broader perspective, the reason we must object to the reform is
that it threatens to bring down the whole education system.

However, there are patients with two negative biopsies who continue with a high
suspicion of cancer, with a persistently elevated PSA or pre-malignant histology report such
as prostate intraepithelial neoplasia grade 3 or atypical microacinar proliferation. The
evidence suggests that cancers diagnosed with the third or fourth biopsy are those of low
grade and volume (Djavan, 2001). The key question is how often is it justified to re-biopsy
the patient that perhaps does not have a cancer that is life threatening.
