Having worked in and traveled to many countries, most recently as the Ambassador
and Permanent Representative of Thailand to the United Nations, I have
seen people in different societies react to trauma in various ways. While experts
contributed to this important book, Trauma Psychology: Issues in Violence, Disaster,
Health, and Illness, I share my perspectives from serving the people of Thailand and
other nations and from my experiences in the aftermath of the 2004 tsunami that
struck the cities and towns in the Indian Ocean rim, including Thailand.
THIS second edition, like the first, is intended to serve as a comprehensive resource tool.

There can be many different variations in how car-sharing is organized. This depends in part
on who is in charge or bears responsibility (a cooperative, a private firm, a municipal
government, or whatever). In addition, car-sharing organizations can be classified as being
either open or closed. A closed car-sharing organization has a set of vehicles used by a limited
group of persons, such as the employees of a firm or of a municipal authority. An open car-
sharing organization, in contrast, is one that in principle anyone can belong to.
Patients who lose healthy bone tissue as a result of inflammation or trauma need bone
regenerative/reconstructive surgery in order to recover the function of the lost bone. To
regenerate the lost bone tissue, autologous bone grafting is the current gold standard,
though this technique is a great burden for patients because transplantable autologous bone
must be harvested from a healthy site, which causes donor site morbidity and pain.

Schools reported the manner in which their nutrition education efforts
are coordinated by a person or group. The majority of public schools
(61 percent) have no nutrition education coordination, meaning each
teacher is responsible for his or her nutrition lessons (table 6). About
9 percent of the schools have one person within the school
coordinating nutrition education; 24 percent coordinate using a group
or committee; and about 6 percent have someone outside the school,
for example from the district, coordinate nutrition education.
Stressful life events such as trauma, loss of a loved one,
a diicult relationship or any stressful situation—whether
welcome or unwelcome—oten occur before a depressive
episode. Additional work and home responsibilities, caring
for children and aging parents, abuse, and poverty also may
trigger a depressive episode. Evidence suggests that women
respond diferently than men to these events, making them
more prone to depression.
Every year an estimated three million newborn babies die during the first week of life due to problems such as sepsis, tetanus, or asphyxia, or problems associated with trauma, low birth weight, or preterm birth. This guide is written for the doctors, nurses, senior midwives, and other health care workers at the first referral level in low resource settings who are responsible for the care of newborn babies with problems during the first week(s) of life.
It is clear from the pages that follow that the physical abuse of one individual by
another, or by both individuals of each other, is an integral component of some
intimate relationships, perhaps many intimate relationships. This book is not about
the violence as much as it is a reflection on our responses to—or our lack of
responses to—that violation of physical integrity and the accompanying emotional
and mental trauma. As these next pages indicate, we all too often fail to recognize
the signs of physical violence.
Total energy expenditure comprises resting energy expenditure (two-thirds) plus activity energy expenditure (one-third) (Chap. 72). Resting energy expenditure includes the calories necessary for basal metabolism at bed rest. Activity energy expenditure represents one-fourth to one-third of the total, and the thermal effect of feeding is about 10% of the total energy expenditure. For normally nourished healthy individuals, the total energy expenditure is about 30– 35 kcal/kg.
Disseminated Intravascular Coagulation DIC is a clinicopathologic syndrome characterized by widespread intravascular fibrin formation in response to excessive blood protease activity that overcomes the natural anticoagulant mechanisms. DIC is associated with several underlying pathologies (Table 110-2). The most common causes are bacterial sepsis, malignant disorders such as solid tumors or acute promyelocytic leukemia (APL), and obstetric causes. DIC is diagnosed in almost half of pregnant women with abruptio placentae or with amniotic fluid embolism.
