People who pursue careers in Internal Medicine are drawn to the specialty by a love of patients, mechanisms, discovery, education, and therapeutics. We love hearing the stories told to us by our patients, linking signs and symptoms to pathophysiology, solving the diagnostic dilemmas, and proposing strategies to prevent and treat illness. It is not surprising given these tendencies that internists prefer to continue their life-long learning through problem solving. This book is offered as a companion to the remarkable 17th edition of Harrison’s Principles of Internal Medicine.

Prevention of Gastrointestinal Illness Diarrhea, the leading cause of illness in travelers (Chap. 122), is usually a short-lived, self-limited condition; however, 40% of affected individuals need to alter their scheduled activities, and another 20% are confined to bed. The most important determinant of risk is the destination. Incidence rates per 2-week stay have been reported to be as low as 8% in industrialized countries and as high as 55% in parts of Africa, Central and South America, and Southeast Asia. Infants and young adults are at particularly high risk.

Diabetes Mellitus Alterations in glucose control and changes in insulin requirements are common problems among patients with diabetes who travel. Changes in time zone, in the amount and timing of food intake, and in physical activity demand vigilant assessment of metabolic control. The traveler with diabetes should pack medication (including a bottle of regular insulin for emergencies), insulin syringes and needles, equipment and supplies for glucose monitoring, and snacks in carryon luggage. Insulin is stable for ~3 months at room temperature but should be kept as cool as possible.

Those who have followed the attempts in Rio de Janeiro, The Hague, Nairobi, Stockholm, Montreal, Kyoto, Buenos Aires, and Johannesburg to construct an international law of the environment have witnessed an intense, active, colorful, fascinating, and often confusing drama. They may have wondered whether the sometimes circuslike sessions involving heads of state and indigenous people and observers actually can produce what they understand to be effective law.

There is no doubt that international law was of major importance during the Gulf conflict of 1990–91. Military and other actions were repeatedly justified through reference to international law, and disputes about interpretation were frequent. This book provides a definitive legal analysis of the conflict, with reference both to international and to English law.

In recent years a burgeoning literature on the apparent ‘fragmentation’ of international law has been developing. 1 It is not a term that has a long history, and is most frequently associated with the problems emerging from the recent proliferation of international courts and tribunals2 and the associated development of autonomous, or semi-autonomous regimes, within the field of international law.

In this paper, a novel and complete process to simulate an active vibration control system in a centerless grinding machine is presented. Based on the updated finite element (FE) model of the machine, the structural modifications performed to incorporate active elements are detailed, as well as the subsequent reduction procedure to obtain a low-order state space model.

Many of the world’s problems—violence, overpopulation, substance abuse, poverty, terrorism, infant mortality, HIV/AIDS, chronic disease— involve human behavior. Since countries are increasingly interdependent, cross-national collaboration is imperative. U.S. psychological scientists can take an active role, working with colleagues in and from other countries, to improve the world’s capacity to address these pressing issues. International research collaboration in the psychological, behavioral, and social sciences is critical to improving the quality of peoples’ lives worldwide.

While Active Learning (AL) has already been shown to markedly reduce the annotation efforts for many sequence labeling tasks compared to random selection, AL remains unconcerned about the internal structure of the selected sequences (typically, sentences). We propose a semisupervised AL approach for sequence labeling where only highly uncertain subsequences are presented to human annotators, while all others in the selected sequences are automatically labeled.
