The rate of antibiotic resistance in Helicobacter pylori (H.pylori) is increasing in Vietnam in recent years and has become a main challenge in the management of H. pylori infection. Data about the resistance remains different depending on the time, demography and especially the methods of determining the antibiotics susceptibility. To determine the antibiogram of H.pylori strains isolated from a population in Central Vietnam to clarithromycin, levofloxacin by E-test and disk diffusion, compare two diffusion methods.

Less than 3 decades ago, Robin Warren and Barry Marshall definitively identified Helicobacter pylori by culturing an organism from gastric biopsy specimens that had been visualized for almost a century by pathologists (196). In 1994, H. pylori was recognized as a type I carcinogen, and now it is considered the most common etiologic agent of infection-related cancers, which represent 5.5% of the global cancer burden (239). In 2005, Marshall and Warren were awarded the Nobel Prize of Medicine for their seminal discovery of this bacterium and its role in peptic ulcer disease

Complications The major clinical complications of PV relate directly to the increase in blood viscosity associated with red cell mass elevation and indirectly to the increased turnover of red cells, leukocytes, and platelets with the attendant increase in uric acid and cytokine production. The latter appears to be responsible for the increase in peptic ulcer disease and for the pruritus associated with this disorder, although formal proof for this has not been obtained. A sudden massive increase in spleen size can be associated with splenic infarction or progressive cachexia.

Peptic Ulcers In addition to clinical features, characteristics of an ulcer at endoscopy provide important prognostic information. One-third of patients with active bleeding or a nonbleeding visible vessel have further bleeding that requires urgent surgery if they are treated conservatively. These patients clearly benefit from endoscopic therapy with bipolar electrocoagulation, heater probe, injection therapy (e.g., absolute alcohol, 1:10,000 epinephrine), and/or clips with reductions in bleeding, hospital stay, mortality rate, and costs.

Other Causes Alkaline reflux esophagitis produces GERD-like symptoms in patients who have had surgery for peptic ulcer disease. Opportunistic fungal or viral esophageal infections may produce heartburn or chest discomfort but more often cause odynophagia. Other causes of esophageal inflammation include eosinophilic esophagitis and pill esophagitis. Biliary colic is in the differential diagnosis of dyspepsia, but most patients with true biliary colic report discrete episodes of right upper quadrant or epigastric pain rather than chronic burning discomfort, nausea, and bloating.

Na+ ions remain in solution even in the presence of HCO3–-rich pancreatic secretions and are subject to absorption, like HCO3–. Because of the uptake of Na+, use of NaHCO3 must be avoided in conditions requiring restriction of NaCl intake, such as hypertension, cardiac failure, and edema. Since food has a buffering effect, antacids are taken between meals (e.g., 1 and 3 h after meals and at bedtime). Nonabsorbable antacids are preferred.
