  • Gynecomastia Gynecomastia refers to enlargement of the male breast. It is caused by excess estrogen action and is usually the result of an increased estrogen/androgen ratio. True gynecomastia is associated with glandular breast tissue that is 4 cm in diameter and often tender. Glandular tissue enlargement should be distinguished from excess adipose tissue: glandular tissue is firmer and contains fibrous-like cords.

  • In children, the symptoms are most often caused by the androgens (male-type hormones) that the tumor might secrete. The most common symptoms are excessive growth of facial and body hair (such as in the pubic and underarm area). The male hormones may also cause enlargement of the penis in boys or the clitoris in girls. If the tumor secretes estrogens (female-type hormones), it can cause girls to start puberty early. This can cause the breasts to develop and menstrual periods to start. In boys, estrogen-producing tumors may cause breast enlargement.

  • Hirsutism, defined as excessive male-pattern hair growth, affects approximately 10% of women. It usually represents a variation of normal hair growth, but rarely it is a harbinger of a serious underlying condition. Hirsutism is often idiopathic but may be caused by conditions associated with androgen excess, such as polycystic ovarian syndrome (PCOS) or congenital adrenal hyperplasia (CAH) (Table 50-1). Cutaneous manifestations commonly associated with hirsutism include acne and male-pattern balding (androgenic alopecia).

  • At the lower end of the uterus is the cervix, a narrow opening. It is through the cervix that the sperm traveled on their way to the egg cell. Connecting the cervix to the outside of the body is the vagina. The vagina is made up of muscular tissue. It is into the vagina that sperm are ejaculated from the penis. Also, the vagina is the birth canal, through which the infant passes as it leaves the mother’s body during childbirth. Unlike in males, the reproductive pathway in females is not combined with the pathway for excretion.

  • In addition to evidence that gender affects growth, evidence from the 2000/2001 World Development Report: Attacking Poverty shows that gender relations affect all aspects of poverty, including income, opportunity, security, and empowerment. 19 With regard to income poverty, in some countries, girls in poor families receive lower quality nutrition, less health care, and poorer education than their brothers. 20 Likewise, as noted above, female household mem- bers often have less access to and control over the household’s pro- ductive resources and income than do male family members.

  • Women leak, inevitably and often bountifully. Menstrual blood, birth fluids, breast milk and sometimes tears lead us to be seen as leakier than men at a physical level. Women are often seen to work through a network of relationships, a web, rather than the hierarchy of male decision-making (Gilligan 1982). Within such a web of relationships, emotions, knowledge and other personal attributes flow, often freely, sometimes unconsciously. Dirt is defined by Mary Douglas as ‘matter out of place’ (Douglas 1966).

  • Spiegel et al. (1989) conducted a prospective intervention study of patients with metastatic breast cancer. Findings demonstrated that a weekly support group meeting for one year enhanced patients’ psychosocial functioning and reduced pain as compared to a standard care treatment control group. Further, in a ten-year follow-up, the data reported that survival was significantly different, with a mean of 36.3 months in the intervention group compared with 18.9 months in the control group. Another study, conducted by Fawzy and colleagues (1993) supported these findings.
