II- Câu hỏi thường gặp trong phần lời khai bệnh
a.Có thể dùng từ tắt SOCRATES như một cách nhớ hữu ích để hỏi về triệu chứng của bệnh nhân:
Site (vị trí): Where exactly is the pain?
Onset (bắt đầu cơn đau): Where did it start?
Character (đặc điểm): What is the pain like?
Radiation (sự lan tỏa): Does it radiate/ move anywhere?
Associated features (đặc trưng kết hợp): Is there anything associated with the pain e.g
sweating, vomitting?
Time course (khoảng thời gian): How long did it last?
Exacerbating/alleviating features (đặc trưng làm tăng/giảm cơn đau): Does anything make it
better/worse?
b.Các câu hỏi về phần lịch sử bệnh án:
1-Have you ever had (headache) before? (nội khoa)
2-Have you had any operations in the past? (ngoại khoa)
3-Have you had any pregnancies? (phụ khoa)
c. Phản ứng phụ và dị ứng (Side-effects and allergies):
