Soft tissue defects of the lower leg and foot are complicated injuries with numerous causes including trauma, ulcers, and Gout. Objectives: 1. To construct a map of the perforators of the peroneal artery and posterior tibial artery using a handheld Doppler ultrasound. 2. To evaluate the clinical outcome of perforator flaps to cover soft tissue defects in the lower leg and foot.

Anterolateral thigh flap is one of the most researched and widely used perforator flaps in the recent decades in plastic surgery as a whole and in limb reconstruction, especially in cases with complex deflects, in particular. This report aimed to evaluate anterolateral thigh flap in reconstruction of complex lower limb soft tissue defects.

Purpose: Soft tissue defects can result in exposition of tendons, bones, joints and nerves… which must be covered early by flaps. Groin free flaps can afford the soft tissue defects. Materials and methods: Groin free flaps were used for 5 cases to cover soft tissue defects at hand, leg, ankle and foot. Those were followed up. Results: 2 soft tissue defects of foot, others 2 of leg and ankle, 1 of hand. All cases were covered by groin free flaps which have good results.
