Ebook "Hot topics in infection and immunity in children V" e was scheduled for 23–25 June 2008 with another exciting programme and renowned speakers and we expect to produce a sixth edition of this book based on that course. Paediatric infectious diseases continue to grow and flourish in Europe with centres now being accredited for training by the European Society for Paediatric Infectious Diseases on behalf of the European Academy of Paediatrics.

The significant social and economic loss as a result of bovine tuberculosis (bTB) presents a continuous challenge to cattle industries in the UK and worldwide. However, host genetic variation in cattle susceptibility to bTB provides an opportunity to select for resistant animals and further understand the genetic mechanisms underlying disease dynamics.

Combination therapy is one of the most effective tools for limiting the emergence of drug resistance in pathogens. Despite the widespread adoption of combination therapy across diseases, drug resistance rates continue to rise, leading to failing treatment regimens.

In the current scenario, the drug-resistant tuberculosis is a significant challenge in the control of tuberculosis worldwide. In order to investigate the in vivo evolution of drug-resistant M. tuberculosis, the present study envisaged sequencing of the draft genomes of 18 serial isolates from four pre-extensively drug-resistant (pre-XDR) tuberculosis patients for continuous genetic alterations.

Continuing evolution of the Mycobacterium tuberculosis (Mtb) complex genomes associated with resistance to anti-tuberculosis drugs is threatening tuberculosis disease control efforts. Both multi- and extensively drug resistant Mtb (MDR and XDR, respectively) are increasing in prevalence, but the full set of Mtb genes involved are not known.

Mixed infections of Mycobacterium tuberculosis and antibiotic heteroresistance continue to complicate tuberculosis (TB) diagnosis and treatment. Detection of mixed infections has been limited to molecular genotyping techniques, which lack the sensitivity and resolution to accurately estimate the multiplicity of TB infections.

Treatment of tuberculous-destroyed lung (TDL) with pleuropulmonary resection is challenging. Pulmonary hemorrhage is a frequent complication of this surgical procedure. Continuous efforts have been made to investigate clinical procedures that may reduce intraoperative bleeding effectively.

WHO’s directly observed therapy (DOT) strategy for tuberculosis (TB) treatment depends upon a well– organized healthcare system. This study sought to evaluate the effectiveness of self-administered drug intake supported by a family member versus in-clinic DOT.

Anti-tuberculosis therapy requires at least six-month treatment with continuous administration of combined antibiotics, including isoniazid, rifampicin, pyrazinamide, and ethambutol. The long-term exposure to antibiotics could cause consequent changes in gut microbiota, which may alter the gastrointestinal function and drug absorption in patients, thereby affect the outcome of treatment.

Pulmonary system is the most commonly involved system in people living with HIV as lungs are continuously exposed to the infection as they are rich in macrophages, dendritic cells, lymphocytes etc. In PLHIV immunity is suppressed, hence lungs are prone for infection and non infectious pulmonary diseases. According to autopsy findings, in HIV lung was affected almost upto 100% in the period of epidemic to 70% in the HAART era. Pneumonia is most common pulmonary manifestation followed by pulmonary tuberculosis and PCP.

Research investment differs from physical investment, because it is difficult to exclude third parties from the assets produced by the research process. As noted in the classic contribution by ARROW (1962), the creator of these assets will typically fail to appropriate most or even all of the social returns it generates. Much of the social returns will accrue as spillovers to com- peting firms and consumers. The appropriation problem is likely to lead to significant under- investment in R&D by private firms (JAFFE et al., 2002).

We conclude that additional investments in cleaner production and products may be stimulated by widening the cost gap between the two types of technologies, for instance, by additionally charging for the use of waste and energy. The potential for continuously substi- tuting end-of-pipe technologies with cleaner technologies might be limited, however, since not all regulations favoring end-of-pipe technologies can be cut down. For example, addi- tional filters currently reduce particulate emissions of Diesel cars more effectively than the more eco-efficient Diesel engines.

Investment of financial and human resources. A higher proportion of national budgets should be allocated to develop- ing adequate infrastructure and services for mental health. At the same time, more human resources are needed to provide care for those with mental disorders and to protect and promote mental health. Countries, especially those with limited resources, need to establish specifically targeted policies, plans and initiatives to promote and support mental health. Who needs to invest? All of us with interest in the health and development of people and communities.
