In the course of updating the earlier research, the authors reviewed the 12 regulatory categories covered in the original report and eliminated three of them (restrictions on portraying children, athletic achievement, and intoxication) because of their limited scope and the difficulty in enforcing them. Harmful alcohol consumption is risky both for the drinker and for other people. An intoxicated person can put people in harm’s way by involving them in traffi c accidents or violent behaviour, or by negatively affecting co-workers, relatives, friends or strangers. A survey in Australia found that two thirds of respondents were adversely affected by someone else’s drinking in the past year. Alcohol consumption also affects society at large.

Methanol (See also Chap. e34) The ingestion of methanol (wood alcohol) causes metabolic acidosis, and its metabolites formaldehyde and formic acid cause severe optic nerve and central nervous system damage. Lactic acid, ketoacids, and other unidentified organic acids may contribute to the acidosis. Due to its low molecular weight (32 Da), an osmolar gap is usually present. Metabolic Acidosis: Treatment This is similar to that for ethylene glycol intoxication, including general supportive measures, fomepizole or ethanol administration, and hemodialysis.

Alcoholic Ketoacidosis: Treatment Extracellular fluid deficits almost always accompany AKA and should be repleted by IV administration of saline and glucose (5% dextrose in 0.9% NaCl). Hypophosphatemia, hypokalemia, and hypomagnesemia may coexist and should be corrected. Hypophosphatemia usually emerges 12–24 h after admission, may be exacerbated by glucose infusion, and, if severe, may induce rhabdomyolysis. Neuromuscular Disease Patients with neuromuscular disease often have an abnormal gait, occasionally as a presenting feature. With distal weakness (peripheral neuropathy) the step height is increased to compensate for foot drop, and the sole of the foot may slap on the floor during weight acceptance. Neuropathy may be associated with a degree of sensory imbalance, as described above. Patients with myopathy or muscular dystrophy more typically exhibit proximal weakness. Weakness of the hip girdle may result in a degree of excess pelvic sway during locomotion.
