We report a 61-year-old male presenting with worsening shortness of breath and 20-pound weight loss. Investigations showed right-sided tumor with interatrial septal wall perforation and left atrial expansion.

Primary cardiac sarcoma is usually derived from the right atrium and it manifests as chest pain, arrhythmia, hemoptysis, dyspnea, and fatigue. There are in principle two tests that may be used in mass screening, PSA (prostate specific antigen) and DRE (digital rectal examination). The PSA test is simple, cheap, safe and acceptable. However the prostatic biopsy, required to investigate positive results, is less acceptable and carries significant risks. The accuracy (sensitivity and specificity) of the PSA test is difficult to determine (4). There is no good standard against which to test it, since prostatic biopsy may itself miss 10% to 30% of cases.

The majority of children with glomerulonephritis (GN) present with proteinuria, hematuria, hypertension, edema, reduced renal function, or the nephrotic syndrome. Post- streptococcal acute GN is familiar to the practicing pedi- atrician. Most affected children have a benign course and can be easily treated by the primary care physician on an ambulatory basis. Obviously, a nephrology consultation should be obtained on patients with oliguria, hyperkalemia, nephrotic syndrome, cardiac overload, and renal insuffi- ciency.

The major pathogenic mechanism of poststreptococcal glomerulonephritis (PSGN) is an in situ immune complex formation due to deposition of streptococcal nephritogenic antigens, such as nephritis-associated plasmin receptor (NAPlr) and Streptococcal pyrogenic exotoxin B (SPE B). Both are capable of activating the alternate pathway of the complement cascade and enhance the expression of adhesion molecules. SPE B also stimulates the production of chemotactic cytokines. NAPlr was isolated from group A streptococcus and was shown to bind plasmin(ogen).

Myogenic Ptosis The causes of myogenic ptosis include myasthenia gravis (Chap. 381) and a number of rare myopathies that manifest with ptosis. The term chronic progressive external ophthalmoplegia refers to a spectrum of systemic diseases caused by mutations of mitochondrial DNA. As the name implies, the most prominent findings are symmetric, slowly progressive ptosis and limitation of eye movements. In general, diplopia is a late symptom because all eye movements are reduced equally. In the Kearns-Sayre variant, retinal pigmentary changes and abnormalities of cardiac conduction develop.
