Historically, it has been found that malignancy is associated with superior vena cava (SVC) syndrome. The past decade has seen more cases of thrombogenic and stenotic SVC syndrome due to increased use of pacemakers and indwelling central lines. As compared to the slowly progressing obstruction in malignancy, rapid thrombogenesis rate and a lack of venous collateral sequelae lead to more acute sequelae in these patients.

In 2009 we changed our institutional guideline for perioperative fluid therapy in patients undergoing extrapleural pneumonectomy (EPP) and implemented the use of central venous oxygen saturation and intended low urine output to guide therapy in the early postoperative period. Assessment of fuid responsiveness is problematic in intensive care unit patients. Lung recruitment maneuvers (LRM) can be used as a functional test to predict fuid responsiveness. We propose a new test to predict fuid responsiveness in mechanically ventilated patients by analyzing the variations in central venous pressure (CVP) and systemic arterial parameters during a prolonged sigh breath LRM without the use of a cardiac output measuring device.

Early goal directed therapy improves survival in patients with septic shock. Central venous pressure (CVP) monitoring is essential to guide adequate resuscitation. Use of peripherally inserted central catheters (PICC) is increasing, but little data exists comparing a PICC to a conventional CVP catheter.
