In the revised Diagnostic and Statistical Manual DSM-5 the definition of personality disorder diagnoses has not been changed from that in the DSM-IV-TR. Every clinician knows that the human relationship with the person diagnosed with a psychotic disorder and particularly schizophrenia is the cornerstone of effective therapy and the foundation for recovery. This was one of the major tenets of an earlier generation of psychotherapeutic effort in schizophrenia and related psychoses but had drifted out of focus during the 1980s with the rise of an excessively narrowbiological psychiatry and the decline of the traditional psychoanalytic approach.

Dissociative Identity Disorder (DID) or multiple personality disorder is assumed to have its onset in childhood, but it is usually diagnosed in the fourth decade. It affects preponderantly women and typically runs a chronic, waxing and waning course. Comorbidities with other conditions (such as mood disorders and substance abuse) and its plethora of clinical manifestations may hinder timely diagnosing.
