Over-the-counter medication overdose is a difficult diagnostic challenge for many physicians as common drug screening assays cannot detect these substances. We present a case of acute psychosis, serotonin syndrome, and anticholinergic overdose-like properties in the setting of Coricidin HBP Cough & Cold tablets, known by their street name Triple-C.

A one-month-old, 30 kg calf was presented to Referral Veterinary Polyclinic ICAR-IVRI, Bareilly, with the history of anorexia, salivation, dullness, ataxia, laboured breathing and administration of bolus having a combination of fenbendazole (3000mg) and ivermectin (100mg). Clinical examination revealed pale mucous membrane, mydriasis, the absence of pupillary and menace reflex, sticky salivation, normal body temperature, tachycardia, tachypnoea, dull, depressed, sudden fall with tremors, abnormal posture and gait.

Sri Lanka has a high suicide rate, with more than 40 % of poisoning admissions due to overdose of drugs with Paracetamol being the commonest. Data regarding cardiotoxicity to paracetamol is very minimal though hepatotoxicity following poisoning is well studied. Paracetamol cardiotoxicity has rarely been clinically significant and may have well been overlooked.
