Many point-of-care laboratory tests are manually entered into the electronic health record by ambulatory clinic staff, but the rate of manual transcription error for this testing is poorly characterized. Using a dataset arising from a duplicated workflow that created a set of paired interfaced and manually entered point-of-care glucose measurements, we found that 260 of 6930 (3.7%) manual entries were discrepant from their interfaced result.

Medication reconciliation (MedRec) can improve patient safety by resolving medication discrepancies. Because information technology (IT) and patient engagement are promising approaches to optimizing MedRec, the SEAMPAT project aims to develop a MedRec IT platform based on two applications: the “patient app” and the “MedRec app.” This study evaluates three dimensions of the usability (efficiency, satisfaction, and effectiveness) and usefulness of the patient app.

The results show a large discrepancy in relative output factor curves found among different collimation systems of the same equivalent field sizes and between the CoPs. The Interagency List of Essential Medical Devices for Reproductive Health was developed by WHO in collaboration with UNICEF and UNFPA and major international and nongovernmental organizations active in the field of reproductive health. In 2004 and 2005, several interagency meetings were held to discuss the discrepancy medical devices existing among the various lists. During these consultations, the decision was taken to develop a common list of devices per category of products and not per range of clinical interventions, to avoid redundancy.

In our search for classics in public health, we found that some works were reprinted several times, had updated versions of the originals or new editions with introductions or commentary, and appeared in different formats in a variety of places. We also found that some works were translated by different individuals, leading to discrepancies in the editions.
