Phenotypic confirmatory test

. This study was carried out on 120 urine specimen collected from catheterized patients, aged between 20-70 yrs. The screening for ESBL production was done by the disc diffusion test as recommended by the Clinical and Laboratory Standards Institute(CLSI) and confirmed by the phenotypic disc confirmatory test (PDCT) and ESBL Hi Crome agar(Hi media-Mumbai).

Beta-lactam antibiotics are the most commonly used antimicrobials against bacterial infections. In recent years, emergence of resistance to these antimicrobial agents due to production of β-lactamases has become a serious global health concern. It was reported as leading cause to ineffectiveness to antibiotic usage, increased morbidity and high cost of treatment. Extended spectrum beta-lactamases (ESBLs) detection amongst the Gram – negative bacilli is considered as an important marker of endemicity.

Antimicrobial resistance is a growing threat worldwide. Increasing resistance to third generation cephalosporins has become a cause for concern. The prevalence of extended spectrum–lactamases (ESBLs) and metallobetalactamases (MBL) among members of Enterobacteriaceae constitutes a serious threat to current -lactam therapy leading to treatment failure. A total of 276 Gram negative isolates were processed. ESBL was detected by phenotypic confirmatory disc diffusion test (PCDDT) using ceftazidime alone and in combination with clavulanic acid.

Pseudomonas aeruginosa is an opportunistic pathogenic bacterium responsible for both acute and chronic infection causing serious infections in patients who are mechanically ventilated individuals, who are immunocompromised, and patients with malignancies or HIV infection. The present study was undertaken with the aim to find prevalence of Metallo-β-Lactamases producing Pseudomonas aeruginosa among clinical isolates by different phenotypic confirmatory methods.

Development of carbapenem resistance is common in Pseudomonas aeruginosa. This resistance, which is due mainly to alteration of the OprD porin, the specific uptake pathway of carbapenems, may also result from acquisition of foreign genes encoding Ambler class A, class B, or class D B-lactamases. These enzymes able to hydrolyze carbapenems at various degrees. Thus, detection of carbapenemase producers in clinical laboratories is of utmost importance for the determination of appropriate therapeutic schemes and the implementation of infection control measures.

Acinetobacter the aerobic non fermenting GNCB are emerging as important pathogens in health care associated infection, exhibiting increased antimicrobial resistance by virtue of multiple resistant mechanisms with practically no therapeutic options. The objective was to determine the multitude of resistant mechanisms by phenotypic and genotypic characterization and the prevalent antimicrobial susceptibility pattern. 175 clinically significant non-duplicate Acinetobacter isolates were included.

Antimicrobial resistance pattern and resistance to third generation cephalosporins has become increased worldwide. The detection of extended spectrum lactamases (ESBLs) and metallobetalactamases (MBLs) among members of Enterobacteriaceae family guide us to use Beta lactam, if not tested leading to treatment failure. One year prospective study, our aim is to detect ESBL and MBL producing gram negative bacilli from various clinical samples. A total of 100 Gram negative bacilli were processed.

Over the period of years infections caused by multidrug resistance organisms has emerged as a major public health problem. Their prevalence rates vary in different parts of the world and hence local data regarding these pathogens were important. Our study was aimed to identify the presence of ESBL and AmpC producing Enterobacteriaceae isolates from various clinical samples in our hospital setup various clinical samples were processed consecutively during the study period for microbiological analysis as per standard operating procedure.

Ventilator-associated pneumonia (VAP) is an important nosocomial infection in mechanically ventilated patients at intensive care unit (ICU). The administration of accurate and timely initial empirical antibiotic therapy is essential to reduce the morbidity and mortality from Ventilator-associated pneumonia. Initial empiric antimicrobial therapy for VAP greatly depends on the type of causative pathogen and its resistance pattern. During the six months study period, 196 patients received mechanical ventilation.
