Plasmacytoma, a localized tumor of monoclonal plasma cells without any clinical, radiological or physical evidence of plasma cell neoplasm (PCN), is a rare entity that accounts for 1% of PCN. Immunoglobulin M (IgM) extramedullary plasmacytoma of mediastinal region has never been reported and is a diagnostic challenge considering other differential diagnoses.

The patient dose incurred from diagnostic procedures during advanced radiotherapy has become an important issue. Many researchers in medical physics are using computational simulations to calculate complex parameters in experiments. However, extended computation times make it difficult for personal computers to run the conventional Monte Carlo method to simulate radiological images with high-flux photons such as images produced by computed tomography (CT).

Within this report, the term “equipment” has been interpreted to mean the main types of equipment used in diagnostic radiology, nuclear medicine and radiotherapy. This follows the precedent established in RP 91 (EC (1997b)). It is important to be aware that treatment of the whole installation is outside the scope of this report. Thus, the requirements for an acceptable physical building with shielding that will adequately protect staff, the public and patients, power supplies and ventilation have not been addressed.

In recent years, the remarkable advances in medical imaging instruments have increased their use considerably for diagnostics as well as planning and follow-up of treatment. Emerging from the fields of radiology, medical physics and engineering, medical imaging no longer simply deals with the technology and interpretation of radiographic images. The limitless possibilities presented by computer science and technology, coupled with engineering advances in signal processing, optics and nuclear medicine have created the vastly expanded field of medical imaging.

It has been an honor and a privilege to chair the committee on the state of science in nuclear medicine. As a diagnostic radiologist, a clinicianscientist, and the chairperson of a large academic radiology department, I have been exposed to the many advances in nuclear medicine and have observed their clinical benefits up close. Participating in this review, however, has allowed me to step back and appreciate the magnitude of the progress that has been achieved, and the crucial role that government funding has played in it.
