This study examines the risk posed to AGYW in a relationship with a “Blesser”, defined as male who provides his female partner with their material needs or desires in exchange for a sexual relationship, an age-disparate (5 or more years older) partner, and the potential compounded risk of being a relationship with a partner or partners who are considered both a “Blesser” and age-disparate.

Both women and men feared infecting a partner or their baby and were anxious about leaving either living or future children as orphans. Women and men were concerned about their ability to financially support their children, given their illness. Women who had given birth to an infected child expressed mixed feelings about becoming pregnant again.

Most HIV-infected women and men had not discussed their fertility desires and intentions with a health care provider because of anticipated negative reactions. Women tended to see doctors and nurses as handling matters related to care and treatment, with the psychosocial aspects being handled by counsellors. Some women who had discussed reproduction with a health care provider found providers to be supportive of their reproductive choice. In Ontario, Canada, Fischer and colleagues (2002) found that 75% of patients had no gender preference, and only 21% strongly felt they desired a female OB/GYN, while 4% wanted a male OB/GYN. Various patient characteristics had no bearing on gender preference, e.g., single, pregnant, those with a history of abortion, STDs or sexual dysfunction. In Israel, Piper and colleagues (2008) found that 60.3% of patients expressed no gender preference for their OB/GYN. Women who had children had a predilection to prefer female OB/GYNs.

Physiology of the Female Sexual Response The female sexual response requires the presence of estrogens. A role for androgens is also likely but less well-established. In the CNS, estrogens and androgens work synergistically to enhance sexual arousal and response. A number of studies report enhanced libido in women during preovulatory phases of the menstrual cycle, suggesting that hormones involved in the ovulatory surge (e.g., estrogens) increase desire. Sexual motivation is heavily influenced by context, including the environment and partner factors.
