Significant bleeding
-
In conventional partial resection of parenchymatous organs significant bleeding is one of the main problems, especially in neonates and infants. The patients rapidly lose considerable amounts of their small blood volume. Laser light leads to photo thermal effects in tissue and causes coagulation, drying up, carbonization and evaporization, depending on the temperature.

Bleeding complications in cardiac surgery may lead to increased morbidity and mortality. Traditional blood coagulation tests are not always suitable to detect rapid changes in the patient's coagulation status.

Spine fusion is a surgical procedure characterized by a significant perioperative bleeding, which often requires red blood cell (RBC) transfusion. Methods: The incidence and the cost of RBC transfusion were evaluated in all patients undergoing elective surgery for spine fusion in our Institution, a high-volume center for spine surgery, over a period of 3 years.

Significant bleeding is a well known complication after cardiac surgical procedures and is associated with worse outcome. Thrombelastometry (ROTEM®) allows point-of-care testing of the coagulation status but only limited data is available yet. The aim was to evaluate the ROTEM®-guided blood component therapy in a randomized trial.

Significant bleeding may occur following endobronchial forceps biopsy or brushing of necrotic or hypervascular tumors in the airways. In some cases, methods such as endobronchial instillation of iced saline lavage and epinephrine may fail to control bleeding.

Although Abnormal Uterine Bleeding (AUB) can have serious medical consequences and significantly impacts daily life, the overall trend is that most women do not seek care for these symptoms. The objective of this review was to synthesise factors impeding women’s access care for AUB.

Venous thromboembolism (VTE) is highly prevalent in cancer patients and can cause severe morbidity. VTE treatment is essential, but anticoagulation increases the risk of major bleeding. The purpose was to evaluate the impact of VTE and major bleeding on survival and to identify significant risk factors for these events in lung cancer patients.

Mild rectal bleeding (MRB) is a particular clinical entity different from necrotizing enterocolitis, which significantly influences neonatal care in preterm infants. We aimed to determine the risk factors and to evaluate prospectively the clinical course of MRB.

Stem bleeding disease caused by Thielaviopsis paradoxa is one of the major diseases of coconut in almost all the coconut growing regions of Karnataaka. Bioefficacy of five botanicals viz. Allium sativa, Nerium olender, Tinospora cordifolia, Osimum sanctum and Aegle marmelos, Seven isolates namely Trichoderma viridae (GKVK), Trichoderma harzianum (GKVK), Trichoderma asperillum (GKVK), Trichoderma harzianum (HRS, NBAII, KRN and MYS) and ten systemic and three contact fungicides were evaluated under in vitro conditions against Thielviopsis paradoxa.

Where a woman gives birth should not decide her fate. Excessive bleeding after childbirth and high blood pressure during pregnancy, two of the leading causes of maternal deaths, can be prevented and significantly reduced with expanded availability of maternal health medicines and supportive policies and practices to achieve scale—a critical action for meeting the Millennium Development Goal (MDG) 5 target: reducing maternal mortality by 75 percent by 2015.

Medicine without statistics is quackery; statistics without medicine is numerology. Perhaps this is the main reason why clinicians should care about statistics. Statistics in medicine began in the early nineteenth century (it was called “the numerical method” then) and its debut involved disproving the most common and widely accepted medical treatment for millennia: bleeding. From ancient Rome until 1900, all physicians – from Galen to Avicenna to Benjamin Rush – strongly and clearly advocated bleeding as the treatment for most medical illnesses.

The aspirator should be aware of the pertinent history and clinical information, significant radiological studies and the clinical question that FNAB may answer. The procedure as well as the minor complications of bruising and bleeding should be explained to the patient. Site-specific complications for deep needle aspiration biopsy should be described to the patient if image guided FNAB is performed. Informed consent should be obtained from the patient, guardian or legal representative.

Oral health professionals play a significant role in counseling patients concerning the harmful effects of tobacco, alcohol and recreational drugs. During the pregnancy, the consequences of these behaviors are profound. Multiple studies have demonstrated a clear association between maternal smoking and perinatal morbidity and mortality (6-10). Women who smoke are at increased risk for low birth weight babies, bleeding during pregnancy, premature labor and preterm rupture of membranes.

Vascular dysfunction may also reduce oxygen delivery to gingival tissue. Pocket oxygen tension was reported to be significantly lower in smokers than non-smokers providing support for the negative effects of smoking on vascular system (Hanioka et al. 2000). Evidence from both human and experimental studies suggests that smoking has a long-term chronic effect, and its effect is not simply a vasoconstriction.

COMBINED ORAL CONTRACEPTIVES Mechanism of action The combined oral contraceptive (COC) pills currently available in the UK are shown in Table 2. They combine an estrogen (ethinylestradiol (EE) in all cases but one) with one of seven progestogens. Aside from secondary contraceptive effects on the cervical mucus and to impede implantation, COCs primarily prevent ovulation. This makes the method highly effective in ‘perfect’ use (Table 1), but it removes the normal menstrual cyle and replaces it with a cycle that is user-produced and based only on the end-organ, i.e. the endometrium.

von Willebrand Disease vWD is the most common inherited bleeding disorder. Estimates from laboratory data suggest a prevalence of approximately 1%, but data based on symptomatic individuals suggest that it is closer to 0.1% of the population. vWF serves two roles: (1) as the major adhesion molecule that tethers the platelet to the exposed subendothelium; and (2) as the binding protein for FVIII, resulting in significant prolongation of the FVIII half-life in circulation.

For patients with a rising PSA after radiation therapy, salvage prostatectomy can be considered if the disease was "curable" at the outset, if persistent disease has been documented by a biopsy of the prostate, and if no metastatic disease is seen on imaging studies. Unfortunately, case selection is poorly defined in most series, and morbidities are significant. As currently performed, virtually all patients are impotent after salvage radical prostatectomy, and ~45% have either total urinary incontinence or stress incontinence.

Approach to the Patient: Gastrointestinal Bleeding Measurement of the heart rate and blood pressure is the best way to assess a patient with GIB. Clinically significant bleeding leads to postural changes in heart rate or blood pressure, tachycardia, and, finally, recumbent hypotension. In contrast, the hemoglobin does not fall immediately with acute GIB, due to proportionate reductions in plasma and red cell volumes (i.e., "people bleed whole blood"). Thus, hemoglobin may be normal or only minimally decreased at the initial presentation of a severe bleeding episode. ...
