The perioperative use and relevance of protective ventilation in surgical patients is being increasingly recognized. Obesity poses particular challenges to adequate mechanical ventilation in addition to surgical constraints, primarily by restricted lung mechanics due to excessive adiposity, frequent respiratory comorbidities (i.e. sleep apnea, asthma), and concerns of postoperative respiratory depression and other pulmonary complications.

Cancer-related fatigue (CRF) and insomnia are major complaints in breast cancer survivors (BC). Aerobic training (AT), the standard therapy for CRF in BC, shows only minor to moderate treatment effects. Other evidence-based treatments include cognitive behavioral therapy, e.g., sleep education/restriction (SE) and mindfulness-based therapies.

Sleep restriction is a prevalent issue for adolescents and has been associated with negative cognitive, emotional, and physical health (e.g., poor attention, depressed mood, obesity). Existing sleep promotion programs are successful in improving adolescents’ sleep knowledge but not sleep behaviour.

Emerging research is helping to place pain and addictive disorders on a continuum rather than on the traditional dichotomy of recent years [12–15]. It is clear to a growing number of clinicians that pain patients can, and sometimes do have concur- rent addictive disorders that decidedly complicate the management of an already challenging patient population [16–19]. It is possible for pain and addiction to exist as comorbid conditions such as the case of the alcoholic with peripheral neuro- pathic pain.

For progress to be maintained in a clinical field like sleep medicine, unimpeded, unrestricted access to data and the advances in clinical practice should be available. The reason this book is exciting is that it breaks down the barriers to dissemination of information. Researchers at the forefront of areas that have limited funding can find it difficult to get data from randomised, double-blind, (placebo-controlled), crossover or parallel group studies, etc., and so may be limited to the lowest level of scientific research, i.e. single case, or restricted observational series....
