An increase in the price of gasoline will increase the usage costs for all automobiles, although the cost-per-mile increase will be greater for less fuel-ecient cars. If there were suciently attractive substitutes for cars as a whole, we might expect across-the-board increases in usage costs to decrease demand for all car models. For what is probably the vast majority of car owners, however, abandoning car ownership entirely and instead relying completely on other forms of transportation is unlikely to be a realistic choice.

In general, evacuation plans are in place for all long term care settings. What varies most is how much has been actually tested, and what sectors have been involved in these drills and exercises. In many cases, the drills have been mostly simulated, the first responders and transportation mechanisms have not been adequately involved, and tracking systems and the mechanics of moving residents with documentation and medications have not been tested. Evacuation plans and drills must be taken to the next level of preparedness by involving all sectors and actually testing as much as possible....

Socioeconomic status, poverty, and race have been found to be correlated with ACSC rates [8-10]. Several studies have examined the associations between ACSCs and demographics using small areas of analysis (typically ZIP code) and have found that ACSCs are higher in lowincome areas and areas with higher concentrations of racial and ethnic minorities [11,12]. The elderly population has not been studied much in this context, because they are thought to be relatively well-insured.

Total GHGEs from the food supply can be split by a nominal boundary of the regional distribution centre (RDC), i.e. primary commodity production and transport to the RDC (pre-RDC) and processing, transport to retail, storage, preparation and waste (post-RDC), in the ratio of 56:44 (Audsley et al. 2009). It is recognised, however, that this is only a nominal boundary as it is not always clear exactly where primary production ends and processing begins for different types of food.

Geriatric case management is a key ingredient of quality health care services for older adults. High rates of chronic conditions, dementia, frailty or disability, and sub-optimal home environment and social supports call for the integration of primary care and case management for elderly patients of community health centers. Case management may support health center disease management efforts but is primarily aimed at supporting the coordination of services that are necessary for living safely in a home environment.

Historically, the organization and design of paid labour have tended to be sex-typed. Equipment, tools and spaces used for paid labour have tended to be designed for men (Courville et al., 1992; Chatigny et al., 1995). Work scheduling has presumed constant availability of the worker, with no constraints arising from responsibility for child care or elder care (Prévost and Messing, 2001). Occupational health and safety standards have often used male models; for example, most toxicological data come from males (Setlow et al., 1998).

In many countries, medicines are provided free to all patients in the public sector. Where this is the case, price data are not reported. In countries where medicines are only provided free to some groups of patients (e.g. children, the elderly), data on the price paid by those who are required to pay for their medicines are collected. In such cases, the price is the full price paid, even if patients themselves only pay part of this price. As indicated in Box 1.1, in WHO/HAI methodology medicine prices are reported as median price ratios or MPRs, which express median local...
