Traumatic brain injury

Although there are over 400,000 people each year in the United States alone who suffer from traumatic injury to the central nervous system (CNS), no phar macological treatment is currently available. Considering the enormity of the problem in terms of human tragedy as well as the economic burden to families and societies alike, it is surprising that so little effort is being made to develop treatments for these disorders.

One of the best attended satellite symposia at the 1995 Neurosurgical Congress in Berlin was on ischaemia in head injury. Its aim, according to its chairman, Professor Francois Cohadon of Bordeaux, was primarily to review the current concept of brain ischaemia in trauma, its diagnosis and its clinical management. Professor Cohadon himself is pre-eminent in the field of accident and emergency surgery in France.

The human personality is inextricably bound up with, among other things, the function of the central nervous system. Diseases and malfunctions of the brain, head injuries and neurosurgical operations can all result in permanently altered behaviour patterns. This interrelation between brain and behaviour is most clearly demonstrated in cases involving functional neurosurgery and severe traumatic lesions.

To determine mortality characteristics of multiple trauma patients who were treated at Military Hospital 103. Methods: A prospective, descriptive study on 196 multiple trauma patients who were admitted and treated at the Surgical Intensive Care Unit (SICU), Military Hospital 103. Patients' clinical and subclinical data were collected at the time of admission, during treatment, and at the time of discharge from the SICU.

Patients with traumatic brain injury often develop sequelae such as eye movement disorders, including diplopia. Eye movement training is effective in diplopia management. However, few longitudinal follow-up studies have been conducted from the subacute disease stage, owing to the complexity of methods for quantifying diplopia.

Traumatic brain injury (TBI) occurs in a large percentage of surgical trauma patients and is one of the leading causes of death amongst young teens and adults. Furthermore, individuals with TBIs often require mechanical ventilation and admission to the intensive care unit.

Survivors of moderate and severe traumatic brain injury typically present with spasticity, an upper motor neuron lesion associated with hyperexcitability of the stretch reflex due to disinhibition of cortical influences on spinal cord circuits and structural and functional changes in skeletal muscle.

The benzodiazepine midazolam is a γ-aminobutyric acid (GABA)-A receptor agonist frequently used for sedation or stress control in patients suffering from traumatic brain injury (TBI). However, experimental studies on benzodiazepines have reported divergent results, raising concerns about its widespread use in patients. Some studies indicate that benzodiazepine-mediated potentiation of GABAergic neurotransmission is detrimental in brain-injured animals.

Procalcitonin (PCT) is a reliable biomarker of sepsis and infection. The level of PCT associated with sepsis and infection in patients with traumatic brain injury is currently unknown. The purpose of this study was to investigate the value of PCT and C-reactive protein (CRP) as diagnostic markers of sepsis and to evaluate the prognostic value of these markers related to the severity of injury, sepsis and mortality.

Intracranial-pressure (ICP) monitoring is considered standard care for severe traumatic brain injury and is used frequently, but the efficacy of treatment based on monitoring in patients with hemorrhagic stroke has not been rigorously assessed. In this study, we investigated the clinical value of ICP monitoring in patients with hemorrhagic stroke.

Intracranial pressure (ICP) monitoring is widely used in the management of patients with severe traumatic brain injury (TBI). However, there is limited evidence about the efficacy of ICP monitoring in older subjects (aged ≥65 years). This study evaluated the effect of intraventricular ICP monitoring on the outcome of older adults suffering from a severe TBI.
