Breathing polluted air is unhealthy. For example, you might find it more difficult to breathe, you might cough or your chest might feel tight. Two main types of air pollution are ozone pollution and particle pollution. The ozone we breathe at ground level is bad. But very high in the atmosphere (the stratosphere) is a natural layer of ozone that is good because it helps protect us from the sun’s harmful ultraviolet rays. A rhyme that can help you remember this is “Ozone: Good up high, bad nearby.

They observed that the use of well defined symptoms as diagnostic tool, even in resource limited settings, may improve the chances of diagnosing childhood pulmonary tuberculosis. Fourie et al observed some clinical criteria thought to be most relevant as predictors of tuberculosis in children . These criteria include history of contact with a case of tuberculosis, positive skin test, persistent cough, low weight for age, and unexplained/ prolonged fever.

Nontypable H. influenzae Nontypable H. influenzae is a common cause of community-acquired bacterial pneumonia in adults. Nontypable H. influenzae pneumonia is especially common among patients with COPD or AIDS. The clinical features of H. influenzae pneumonia are similar to those of other types of bacterial pneumonia (including pneumococcal pneumonia). Patients present with fever, cough, and purulent sputum, usually of several days' duration. Chest radiography reveals alveolar infiltrates in a patchy or lobar distribution.

Clinical Manifestations Lung cancer gives rise to signs and symptoms caused by local tumor growth, invasion or obstruction of adjacent structures, growth in regional nodes through lymphatic spread, growth in distant metastatic sites after hematogenous dissemination, and remote effects of tumor products (paraneoplastic syndromes) (Chaps. 96 and 97). Although 5–15% of patients with lung cancer are identified while they are asymptomatic, usually as a result of a routine chest radiograph or through the use of screening CT scans, most patients present with some sign or symptom.

Approach to the Patient: Hemoptysis The history is extremely valuable. Hemoptysis that is described as bloodstreaking of mucopurulent or purulent sputum often suggests bronchitis. Chronic production of sputum with a recent change in quantity or appearance favors an acute exacerbation of chronic bronchitis. Fever or chills accompanying bloodstreaked purulent sputum suggests pneumonia, whereas a putrid smell to the sputum raises the possibility of lung abscess. When sputum production has been chronic and copious, the diagnosis of bronchiectasis should be considered.
