The Lililwan Project is the first population-based study to determine Fetal Alcohol Spectrum Disorders (FASD) prevalence in Australia and was conducted in the remote Fitzroy Valley in North Western Australia. The diagnostic process for FASD requires accurate assessment of gross and fine motor functioning using standardised cut-offs for impairment.

Most Australian Aboriginal children are on track with their development, however, the prevalence of children at risk of or with a developmental or behavioural problem is higher than in other children. Aboriginal child development data mostly comes from remote communities, whereas most Aboriginal children live in urban settings.

Aboriginal children and recent immigrants are more likely than other Canadian children to grow up poor. (67) Furthermore, the effects of the socio-economic gradient on health status remain throughout the life cycle, such that “children who grow up in low socio-economic circumstances but move up the socio-economic ladder during adulthood, are likely to experience physical and mental health problems that remain infl uenced by their childhood socio-economic status.

On Milk Street, in Boston, opposite the Old South Church, lived Josiah Franklin, a maker of soap and candles. He had come to Boston with his wife about the year 1682 from the parish of Ecton, Northamptonshire, England, where his family had lived on a small freehold for about three hundred years. His English wife had died, leaving him seven children, and he had married a colonial girl, Abiah Folger, whose father, Peter Folger, was a man of some note in early Massachusetts.

The impact of undernutrition on young girls has received special attention. In many parts of the world, poverty often interacts with sociocultural factors that make girls and adult women less favoured than men. Female infants and children commonly receive less medical care and also less and lower-quality food than male children.

Parents and children need a framework for care which provides continuity from pre-pregnancy, through pregnancy and childbirth, to the early years of life. A comprehensive approach to early life is needed which builds on existing programmes to ensure our children get the best start in life8. Midwives have a key role in ensuring that their contribution integrates with the roles of other professionals and agencies working in collaboration with maternity services.

Health Disparities Across the continent, significant health disparities exist. In Canada, Aboriginal children rank with many children in the developing world on several key indicators, including infant mortality and injury deaths. In Mexico, children living in rural and indigenous communities experience worse health outcomes than those who live in cities. And, in the United States, children of color suffer poorer health on a number of indicators. Children’s health and security demand our attention.

Besides the previously discussed papers about impacts of shocks at birth, our results are related to research on gender bias during early childhood. Much of the literature finds evidence favoring boys over girls (see Rose (1999) for evidence from India that gender bias in infant mortality drops significantly when districts experience higher rainfall or Dercon and Krishnan (2000) for evidence from Ethiopia that poor households are unable to smooth their consumption, with women bearing the brunt of adverse shocks).

together we must make a decisive move, now, to improve the health of women and children around the world. we know what works. we have achieved excellent progress in a short time in some countries. the answers lie in building our collective resolve to ensure universal access to essential health services and proven, life-saving interventions as we work to strengthen health systems. these range from family planning and making childbirth safe, to increasing access to vaccines and treatment for hIV and aIds, malaria, tuberculosis, pneumonia and other neglected diseases.
