Ebook "Pulmonary care and clinical medicine" examines a range of contemporary ideas and arguments concerning the core yet unsettled clinical issues. Important aspects of pulmonary disorders are tackled such as occupational and environmental respiratory health hazards, the role of vitamin D supplementation in airway allergy, or nutritional aspects of care for lung cancer patients.

A new supraglottic device, the LMA-Supreme™, has recently become available for clinical use. Information on anaesthetic and co-adjuvant requirements for insertion of the LMA-Supreme™ is limited. The present study aimed to evaluate the optimal effect-site concentration of propofol in 50 % (EC50) of adults necessary for successful insertion of the LMA-Supreme™ and to examine remifentanil’s effect on propofol requirements.

In trauma patients intubated in a physician-led pre-hospital trauma service we prospectively examined the rate of misplaced tracheal tubes, the presence and nature of gross airway contamination, and the value of ‘quick look’ airway assessment to identify patients with subsequent difficult laryngoscopy.

Endotracheal intubation of patients with massive regurgitation represents a challenge in emergency airway management. Gastric contents tend to block suction catheters, and few treatment alternatives exist. Based on a technique that was successfully applied in our district, we wanted to examine if endotracheal intubation would be easier and quicker to perform when the patient is turned over to a semiprone position, as compared to the supine position.

A number of studies have examined the association between mold exposure and childhood asthma. However, the conclusions were inconsistent, which might be partly attributable to the lack of consideration of gene function, especially the key genes afecting the pathogenesis of childhood asthma.

Few studies have examined the relationships between sputum inflammatory markers and subsequent annual decline in forced expiratory volume in 1 s (dFEV1). This study investigated whether indices of airway inflammation are predictors of dFEV1 in a general population-based sample.

We investigated patients with chronic obstructive pulmonary disease (COPD) to analyze patterns and identify determinants of healthcare use, according to the severity of airflow obstruction. We used retrospective cohort data from a combination of the 4th Korea National Health and Nutritional Examination Survey (KNHANES) and Korean National Health Insurance (NHI) claims.

Compliance with CPAP treatment for OSAS is not reliably predicted by the severity of symptoms or physiological variables. We examined a range of factors which could be measured before CPAP initiation to look for predictors of compliance.

Obstructive sleep apnea (OSA) is prevalent in individuals with Osteogenesis imperfecta (OI). To date, no study has investigated treatment of OSA in adult individuals with OI using positive airway pressure (PAP). This observational pilot study examined the adherence of adults with OI to treatment of OSA with PAP therapy, and the evolution of self-experienced sleepiness and depression symptoms before and after treatment.

The prevalence of asthma is sharply increasing in pediatric population and has become a global burden of medicine and economics. Nevertheless, evidence for the diagnostic criteria of this condition is still lacking. This condition leads to over-diagnosis of asthma in cases, especially younger children. Stridor and wheezing, pathologic signs of airway obstruction, are sometimes mistakenly identified. As a result, disorders of extra-thoracic airway obstruction may be misdiagnosed with intrathoracic diseases.

Mannitol- and exercise bronchial provocation tests are both used to diagnose exercise-induced bronchoconstriction. The study aim was to compare the short-term treatment response to budesonide and montelukast on airway hyperresponsiveness to mannitol challenge test and to exercise challenge test in children and adolescents with exercise-induced bronchoconstriction.

Neonatal inspiratory stridor is an important examination finding that requires immediate and adequate evaluation of the underlying etiology. Depending on the severity of the airway obstruction and the presence or absence of associated symptoms such as respiratory distress and feeding problems, early initiation of a complete diagnostic workup can be crucial.

Dyspnea is the subjective sensation of uncomfortable or difficult breathing. Most patients report dyspnea as "shortness of breath." Patients report dyspnea when their breathing is excessive for the activity that they are doing. The sensation of dyspnea is produced by stimulation of both central and peripheral receptors that monitor respiratory muscle activity, hypoxia, hypercapnia, acid-base status, airway irritation, and changes in the pressure volume characteristics of the lung (i.e., j receptors in lung fibrosis or emphysema).
