In Sweden, airway guidelines aimed toward improving patient safety have been recommended by the Swedish Society of Anaesthesia and Intensive Care Medicine. Adherence to evidence-based airway guidelines is known to be generally poor in Sweden. The aim of this study was to determine whether airway guidelines are present in Swedish anaesthesia departments.

Airway guidelines recommend an emergency surgical airway as a potential life-saving treatment in a “Can’t Intubate, Can’t Oxygenate” (CICO) situation. Surgical airways can be achieved either through a cricothyroidotomy or tracheostomy. The current literature has limited data regarding complications of cricothyroidotomy and tracheostomy in an emergency situation.

There is a major paradigm shift for intraoperative mechanical ventilator support by the introduction of lung protective ventilation strategies to reduce postoperative pulmonary complications and improve overall clinical outcomes in non-thoracic surgeries. However, there is currently a lack of standardized practice guideline for lung protection during thoracic surgeries that require one-lung ventilation (OLV). Current practice guidelines for ventilatory management advocate protective lung strategies to prevent volutrauma, barotrauma, atelectrauma and biotrauma (3-5). The principles are to use low tidal volumes (Vt) (5-6 ml/kg ideal body weight), maintenance of low mean airway pressures ≤ 30 cmH2O, judicious use of positive end-expiratory pressure (PEEP) with Δ pressure ≤ 18 cmH2O, higher respiratory rates and permissive hypercapnia.
