Airway maintenance

After prehospital insertion of i-gel, a popular supraglottic airway (SGA), fiberoptic-guided intubation through i-gel is often required to switch the i-gel to a definitive airway for anticipated difficult airway. The Aintree intubation catheter (AIC) was developed for this purpose yet it requires many procedural steps during which maintenance of adequate ventilation is difficult.

The dead space washout and provision of some level of positive airway pressure by nasal high-flow (NHF) cannula could improve the efficiency of ventilation, oxygenation and maintenance of the upper airway in patients undergoing deep sedation.

Gas monitoring lines are often the cause of a significant leak; check that they are properly attached and any sampling ports not in use have been blanked off. To eliminate the need to change the sampling line repeatedly, the gas monitoring line should be assembled as an integral part of the breathing circuit by attaching it proximal to the patient breathing filter. Airway equipment These include bacterial filters, catheter mounts, connectors and tracheal tubes, laryngeal mask airways, etc.

Current practice guidelines for ventilatory management advocate protective lung strategies to prevent volutrauma, barotrauma, atelectrauma and biotrauma (3-5). The principles are to use low tidal volumes (Vt) (5-6 ml/kg ideal body weight), maintenance of low mean airway pressures ≤ 30 cmH2O, judicious use of positive end-expiratory pressure (PEEP) with Δ pressure ≤ 18 cmH2O, higher respiratory rates and permissive hypercapnia.
