Two of the most commonly prescribed medication classes in outpatient settings with frequent rates of potentially inappropriate prescribing include antibiotics and nonsteroidal anti-infammatory drugs (NSAIDs).

Faced with national requirements to promote antimicrobial stewardship and reduce drug-resistant infections, community hospitals are challenged to make the best use of existing resources. Eighteen months after building antibiotic decision support into our electronic order platform, high-risk antibiotic use decreased by 83% (P < .001) at our community hospital. Hospital-acquired Clostridium difficile infections declined 24% (P ¼ .07).

The increasing threat of bacterial resistance promotes the need for antibiotic stewardship programs to foster responsible antimicrobial use. Therefore, guidelines for prudent use supported by an online stewardship tool (AntibioticScout.ch) were introduced in Switzerland in December 2016. They recommend (with decreasing preference) a first, second or third line antimicrobial for treatment.

The present study confirms the multi-drug resistant nature of Acinetobacter species, and highlights the correlation between its incidence and duration of hospitalization. Rigorous use of standard precautions and minimizing hospital stay would reduce infection, and consequently, antibiotic overuse, which contributes further to drug resistance. In addition, a robust antibiotic policy and antibiotic stewardship in the hospital can minimize its incidence.

Urinary tract infections, caused by multidrug resistant uropathogenic Escherichia coli (UPEC) are alarmingly escalating. Widespread utilization of empirical antibiotics without performing anti-microbial susceptibility testing led to expanding resistance to different classes of antimicrobial agents generally prescribed among UPEC isolates in our country. Staphylococcus haemolyticus is part of the human normal skin microflora and is also found in nonhuman primates. This organism has been documented as a cause of primary and nosocomial bacteremia in clinical health care settings. Though CONS is ruled out as a contaminant, the emergence of multidrug-resistant isolates to glycopeptides, methicillin, fluoroquinolones, macrolides, lincosamides, streptogramin B and linezolid results in treatment failure in primary bacteremia.

Acinetobacter baumannii has emerged as a worldwide problem as a nosocomial pathogen in hospitalized patients. Acinetobacter spp. can cause a multitude of infections including pneumonia, bacteremia, meningitis, urinary tract infections, and skin and soft tissue infections, and the mortality associated with these infections is high. Isolates resistant to almost all commercially available antimicrobials have been identified, thus limiting treatment options.
