Ebook "Chemistry of antibiotics and related drugs" builds on the success of the earlier edition, offering alternative strategies for discovering new antibiotics. It discusses how the various types of antibiotics and related drugs work to cure infections. Then it delves into the very serious matter of how bacteria are becoming resistant to these antibiotics. It also covers the global action plan on antimicrobial resistance from the World Health Organization and discusses several Antibiotic Stewardship Programs adopted by agencies at local levels.

Faced with national requirements to promote antimicrobial stewardship and reduce drug-resistant infections, community hospitals are challenged to make the best use of existing resources. Eighteen months after building antibiotic decision support into our electronic order platform, high-risk antibiotic use decreased by 83% (P < .001) at our community hospital. Hospital-acquired Clostridium difficile infections declined 24% (P ¼ .07).

Nucleic acid amplification technologies (NAAT) are advancing our ability to make rapid molecular diagnoses in patients with serious culture negative infections. This is the first report of PCR coupled to electrospray ionization mass spectrometry use in the evaluation of complicated community acquired pneumonia in a pediatric patient.

Avoidance of unnecessary antimicrobial administration is a key tenet of antimicrobial stewardship; knowing the optimal duration of therapy obviates over-treatment. However, little research has been performed to establish course lengths for common canine infections. In clinical practice, antimicrobial therapy is frequently prescribed in dogs presenting lower urinary tract signs (haematuria, pollakiuria and dysuria/stranguria).

The increasing threat of bacterial resistance promotes the need for antibiotic stewardship programs to foster responsible antimicrobial use. Therefore, guidelines for prudent use supported by an online stewardship tool (AntibioticScout.ch) were introduced in Switzerland in December 2016. They recommend (with decreasing preference) a first, second or third line antimicrobial for treatment.

Antimicrobial stewardship activities are essential to improve prudent antimicrobial use. The aim of the present study was to evaluate changes in antimicrobial prescriptions in cats after the introduction of prudent use guidelines promoted by an online antimicrobial stewardship tool (AntibioticScout.ch) in Switzerland.

The microbiology and the susceptibility patterns of infected total knee arthroplasties (TKAs) vary depending on demographic, local antimicrobial stewardship, and surgical factors. We wanted to compare the recent microbial profile and antimicrobial resistance pattern in revisions due to infections after primary TKAs in Sweden and Lithuania.

The study showed an increased prevalence of MRSA among the inpatients when compared to the outpatients. A single intervention will unlikely to have a significant impact on attenuating MRSA infection rates. Multifactorial bundled approach is the need of the hour in containing the MRSA infections along with robust antimicrobial stewardship and prompt infection control measures and judicious use of the drugs of choice for MRSA infection which have to be preserved for the future use.

Antimicrobial resistance is a current and fore coming worldwide problematic issue. It is more prevalent in healthcare settings; especially in hospital intensive care units. This study aimed to present a simple means of constructing an antibiotic policy from results of cumulative antibiograms for patients’ cultures [sputum, urine, blood, wounds] using a battery of narrow and broad spectrum antibiotics.

Urinary tract infections, caused by multidrug resistant uropathogenic Escherichia coli (UPEC) are alarmingly escalating. Widespread utilization of empirical antibiotics without performing anti-microbial susceptibility testing led to expanding resistance to different classes of antimicrobial agents generally prescribed among UPEC isolates in our country. Therefore, this study aims to detect the prevalence and the antibiotic susceptibility pattern of UPEC isolates, in Tanta University Hospitals, Egypt. A total of 200 urine samples were collected from July 2015 to August 2016.

Community-acquired pneumonia (CAP) is a common cause of pediatric admission to hospital. The objectives of this study were twofold: 1) to describe the clinical characteristics of CAP in children admitted to a tertiary care pediatric hospital in the pneumococcal vaccination era and, 2) to examine the antimicrobial selection in hospital and on discharge.

Prompt initiation of empiric therapy is common practice in case of suspected meningitis or encephalitis. However, in children the most common pathogens are viruses that usually do not require and are not covered by the applied anti-infective treatment. Novel multiplex PCR (mPCR) panels provide rapid on-site diagnostic testing for a variety of pathogens.

Group A streptococcus (GAS) is the most common bacterial etiology of pharyngitis but is difficult to distinguish clinically from viral pharyngitis. There are benefits to early antibacterial treatment of GAS pharyngitis, but administering antibiotics to children with viral pharyngitis is ineffective and costly.
