Sildenafil is a selective phosphodiesterase type 5 inhibitor used for the treatment of erectile dysfunction and pulmonary hypertension. It is available over the counter in many countries. While there have been a few reports of retinal vascular occlusion following sildenafil consumption, most cases have other comorbidities as risk factors for the disease, and the exact causal role of this drug in these conditions remains unclear.

Ischemic or volatile anesthetic preconditioning is defined as tissue protection from impending ischemic cell damage by repetitive short periods of tissue exposure to ischemia or volatile anesthetics. In 1856, Rudolf Virchow published “Cellular pathology” based on macroscopic and microscopic observation of diseases, and described a triad of factors on thrombosis. The three components were vascular change, blood flow alteration, and abnormalities of blood constituents. Although Virchow originally referred to venous thrombosis, the theory can also be applied to arterial thrombosis, and it is considered that atherothrombus formation is regulated by the thrombogenicity of exposed plaque contents, local hemorheology, and blood factors.
