Bài viết trình bày đánh giá hiệu quả của VA ECMO trong điều trị viêm cơ tim mất bù ở trẻ em. Đối tượng, phương pháp: Nghiên cứu mô tả hồi cứu một loạt ca bệnh gồm 40 bệnh nhi từ 1 tháng đến 16 tuổi được chẩn đoán viêm cơ tim cấp sốc tim, được điều trị bằng VA ECMO tại khoa Điều trị tích cực Nội khoa – Bệnh viện Nhi Trung Ương trong thời gian từ 01/01/2016 đến 30/09/2021.

Extracorporeal membrane oxygenation (ECMO) can be used as a bridge to medical therapy in decompensated right heart failure due to pulmonary arterial hypertension (PAH). A middle-aged man with previously undiagnosed pulmonary hypertension was successfully bridged to intravenous prostanoid therapy with venoarterial ECMO support after presenting with cardiogenic shock and hypoxemic respiratory failure.

In this observational study near infrared spectroscopy (NIRS) was evaluated as a non-invasive monitor of impaired tissue oxygenation (StO2) after cardiac surgery. StO2, cardiac output, mixed venous oxygen saturation and mean arterial pressure were compared with lactate clearance as established measure for sufficient tissue perfusion and oxygen metabolism.

Increased lung water and the resultant atelectasis are significant pulmonary complications after cardiopulmonary bypass (CPB) in children undergoing cardiac surgery; these complications are observed after CPB than after anaesthesia alone.
