This report describes a 2-year prospective, longitudinal survey of attending physicians in 3 clinical areas (family medicine, general pediatrics, internal medicine) who experienced a transition from a homegrown electronic health record (EHR) to a vendor EHR. Participants were already highly familiar with using EHRs.

Non-attendance at gynecological clinics is a major limitation of cervical cancer screening and self-collection of samples may improve this situation. Although HPV testing of self-collected vaginal samples is acceptable, the specificity is inadequate. The current focus is increasing self-collection of vaginal samples to minimize clinic visits.

It demands of us that we attend to the enormous unmet needs in child and adolescent mental health, that we recognize the paucity of services precisely where needs are greatest, and that we insist on action to remedy the treatment gap. Some 30 years ago, Julian Tudor Hart, a primary care physician practicing in a low income community in Wales, proposed an inverse care law. Some small studies have also investigated the utility of ultrasound contrast agents to detect active bleeding [2]. Ultrasound is relatively sensitive for free abdominal fluid.
