Low adherence to bisphosphonate therapy is associated with increased fracture risk. Factors associated with discontinuation of osteoporosis medications have not been studied in-depth. This study assessed medication discontinuation and switching patterns among Medicare beneficiaries who were new users of bisphosphonates and evaluated factors possibly associated with discontinuation.

Bisphosphonate (BP) therapy has been associated with atypical femur fracture (AFF). However, the threshold of treatment duration leading to increased AFF risk is unclear. In a retrospective cohort of older women initiating BP, we compared the AFF risk associated with treatment for at least three years to the risk associated with treatment less than three years.

Atypical fracture of the ulna due to SSBT is a rare entity; there is no standardized treatment strategy for this condition.

Neoadjuvant and concurrent luteinising hormone-releasing hormone agonist (LHRHa) therapy is recommended for 3 to 6 months in men receiving radical radiotherapy for locally advanced prostate cancer. Adjuvant hormonal therapy in addition to radical prostatectomy is not recommended, even in men with margin-positive disease, other than in the context of a clinical trial 10 . Adjuvant hormonal therapy is recommended for a minimum of 2 years in men receiving radi- cal radiotherapy for locally advanced prostate cancer who have a Gleason score of ≥ 8.

The ideal mode of palliation (symptom control) is the removal or minimisation of the cause (i.e. disease-directed therapies). For example, in malignant bone pain, surgery, chemotherapy, radiotherapy and/or bisphosphonates may be used. For an infection, antimicrobials or surgical drainage of an abscess may be required. Alongside disease directed therapy, there are a host of pharmacological and non-pharmacological therapies, which should be used on an individual basis depending on the specific clinical situation.
