Bone marrow edema of the sacroiliac joint is the early imaging manifestation, an indicator of infamma‑ tory activity of ankylosing spondylitis (AS) (Yang R, et. al. Medicine (Baltimore) 98:e14620, 2019). The aim of the study was to investigate the value of magnetic resonance imaging (MRI) Dixon sequence in the diagnosis of marrow edema of the sacroiliac joint in patients with AS.

At present, there are no registered products for the treatment of subchondral Bone Marrow Edema Lesion (BML) and associated knee pain. Patients who do not respond to current anti-inflammatory therapies are left with limited treatment options, and may resort to operative management with Total Knee Arthroplasty (TKA).

Psoriatic Arthritis (PsA) is a chronic inflammatory arthritis that develops in patients with psoriasis. Inflammatory edema in the spine may reflect subclinical disease activity and be a predictor of radiographic progression.

If bone union is expected, conservative treatment is generally selected for lumbar spondylolysis. However, sometimes conservative treatments are unsuccessful. We sought to determine the factors associated with failure of bony union in acute unilateral lumbar spondylolysis with bone marrow edema including contralateral pseudarthrosis.

Type 2 diabetes mellitus is a serious public health problem in the world and its treatment is still a major therapy challenge. Thiazolidinediones (TZDs) are a class of antidiabetic drugs that are used widely to increase insulin sensitivity by activating PeroxisomeProliferator-Activated-Receptor-gamma (PPARγ). Activated PPARγ makes critical alterations in the transcription of genes that regulate glucose and lipid metabolism. However, TZDs have side effects, such as weight gain, edema, congestive heart failure, and bone fracture.

Transient osteoporosis of the hip (TOH) is a rare and temporary clinical condition characterised by bone marrow edema (BME), severe pain, and functional limitation. It commonly occurs in middle-aged men or in women in the last trimester of pregnancy.

Modic Changes (MCs, magnetic resonance imaging (MRI) signal changes in the vertebral bone marrow extending from the vertebral endplate) may represent a subgroup of nonspecific chronic low back pain that could benefit from a specific management.

In some patients ankylosing spondylitis is restricted exclusively to an affection of the sacro- iliac articulation (sacroiliitis), best shown by MRI, as it causes bone marrow edema and cartilage changes. X-ray takes a long time to reveal this arthritis, because there only bone destruction and ankylosis are visible, these are not seen in the early stages of the disease. In the end sacroiliitis can lead to total destruction and ankylosis of the sacroiliacal joint, then of course it is clearly visible in x-ray (Figure 3).

The biological changes underlying the transition process from gingival health to early inflammatory changes involve local increase in vascular permeability, edema and the recruitment and activation of polymorphonuclear neutrophils (PMN) (Delima and Van Dyke 2003). Acquired immune response becomes involved once antigen-presenting cells interact with immunocompetent cells, such as T and B lymphocytes, leading to the expansion of antibody-secreting plasma cells and the development of the chronic lesion (Gemmell and Seymour 2004).

Discoid lupus erythematosus. Violaceous, hyperpigmented, atrophic plaques, often with evidence of follicular plugging, which may result in scarring, are characteristic of discoid lupus erythematosus (also called chronic cutaneous lupus erythematosus). Scleroderma and Morphea The skin changes of scleroderma (Chap. 316) usually begin on the hands, feet, and face, with episodes of recurrent nonpitting edema.

Differential Diagnosis Dyspnea is the consequence of deviations from normal function in the cardiopulmonary systems. Alterations in the respiratory system can be considered in the context of the controller (stimulation of breathing); the ventilatory pump (the bones and muscles that form the chest wall, the airways, and the pleura); and the gas exchanger (the alveoli, pulmonary vasculature, and surrounding lung parenchyma). Similarly, alterations in the cardiovascular system can be grouped into three categories: conditions associated with high, normal, and low cardiac output (Fig. 33-2).
